健脾理气方对慢性萎缩性胃炎癌前病变中Survivin与Fas表达的影响

摘　　要：目的:观察健脾理气方对慢性萎缩性胃炎癌前病变的临床疗效及其对凋亡基因Survivin与Fas表达的影响。方法:66例患者按按完全随机化法分为两组,治疗组和对照组各33例。治疗组口服健脾理气方,对照组口服胃复春片,两组均治疗12周。观察两组患者的综合疗效、临床症状、胃镜情况、病理情况、幽门螺旋杆菌(Hp)根除率及Survivin与Fas表达情况。结果:治疗组的综合疗效、临床症状积分、胃镜情况、病理评分和Hp根除率均优于对照组,且可降低Survivin的表达与提高Fas的表达,两组比较差异有统计学意义(P<0.05)。结论:健脾理气方可以明显改善慢性萎缩性胃炎癌前病变患者的临床症状,减轻胃黏膜肠上皮化生和不典型增生程度,其中降低Survivin的表达与提高的Fas表达可能为其机制之一。Objective： To observe the clinical effect and the expression of Survivin and Fas of Jianpi Liqi Formula on patients with precancerous lesions of chronic atrophic gastritis. Method： 66 patients with precancerous lesions of chronic atrophic gastritis were equally randomized into two groups, which were the control group and the treatment group （ n^33 ） . In the treatment group Jianpi Liqi Formula decoction was given to the patients and in the control group Weifuchun tablets was given. Treated for 12 weeks, the total effective rate, clinical symptoms, gastroscopic images, pathological changes of gastric mucous membranes, Hp-eradicating rates , the expression of Survivin and Fas were observed. Result： The total effective rate, clinical symptoms, gastroscopic images, pathological changes of gastric mucous membranes and Hp-eradicating rates of the treatment group was better than that of the contr01 group （ P〈0.05 ） . The expression of Survivin was suppressed and the expression of Fas was enhanced in the treatment group （ P〈O.05 ） . Conclusion： Jianpi Liqi Formula can improve the symptoms in patients with precancerous lesions of chronic atrophic gastritis and prevent the development of precancerous lesions of chronic atrophic gastritis. The mechanism of the clinical effect is probably due to down-regulate Survivin expression and up-regulate Fas expression.
