关于100例头位难产的临床观察与分析

摘　　要：目的：本文主要探讨造成头位难产原因以及临床的表现和诊疗方法等，确保降低剖宫产实施率，减少母婴的并发症等。方法选取我院2011年1月-2013年1月接受治疗的100例头位难产患者资料开展回顾分析，通过临床观察，并对患者情况进行分析，总结梳理头位难产原因、诊断依据及治疗方法。结果经过对100例头位难产的准确诊断和有效诊疗，孕产妇中有64例实施剖宫产，有27例孕产妇接受阴道助产，自然分娩孕产妇有9例。对新生儿开展Apgar的评分，结果显示有72例新生儿在8分以上，25例新生儿处于4-7分，另有3例新生儿在3分以下，产妇和新生儿未发生围生产期死亡。结论通过临床上对头位难产产妇的密切观察，及时掌握产程中头位难产的特征并准确判断头位难产，采取有效措施引导胎头下降，注重处理产力以及胎头的位置等可变因素，尽量促使难产转变为顺产，减少剖宫产实施率和母子并发症。Objective This paper mainly discusses the cause head during childbirth and performance and clinical diagnosis and treatment method, etc., to ensure that been reducing cesarean delivery, reduce maternal and infant complications, etc.Methods Select our hospital between January 2011 and January 2013 during the treatment of 100 patients with head dystocia data to conduct retrospective analysis, through clinical observation, and analyzes the clinical situation, summarizes the comb head dystocia causes, diagnosis and treatment.Results Of 100 cases of head dystocia accurate diagnosis and effective diagnosis and treatment of 64 cases of cesarean delivery, maternal 27 cases of maternal vaginal midwifery, natural birth maternal in 9 cases. According to the result of the neonatal score of Apgar, there are more than 72 newborns in 8 points, 25 infants between 4-7 points, otherwise 3 infants under 3 points, maternal and neonatal death was not occurred around the production period.Conclusion By clinical adversary during childbirth puerpera close observation, timely grasp the features of the labor head during childbirth and accurate judgment head dystocia, take effective measures to guide the fetal head down, pay attention to the processing productivity and variable factors such as fetal head position, try to encourage childbirth into natural labor, reduce cesarean delivery been complications and mother and son.
