高龄高血压患者血清胱抑素C与左心室肥厚及心功能的相关性研究

摘　　要：目的分析高龄高血压患者血清胱抑素C（CysC）水平与左心室肥厚及心功能的相关性。方法将110例高龄高血压患者根据左心室质量、NYHA心功能分级分为左心室肥厚组（LVH组）与非肥厚组（非LVH组）,比较两组血清CysC水平及与心功能之间的相关性。结果 LVH组CysC水平较非LVH组明显升高,差异有统计学意义（P〈0.01）。血清CysC水平与左心室舒张末期内径（LVDD）、室间隔厚度（IVS）、左心室后壁厚度（LVPW）及左心室质量指数（LVMI）呈正相关（r=0.27-0.35,P〈0.01）。CysC与射血分数（EF）值负相关（r=-0.39,P〈0.01）,与脑钠肽（BNP）水平呈正相关（r=0.29,P〈0.01）。CysC水平与心功能分级间呈正相关（r=0.56,P〈0.01）。结论高龄高血压患者血清CysC水平与左心室肥厚及心功能密切相关,可作为反映左心室肥厚程度及心功能分级的重要指标之一。Objective To study the correlation among the levels of serum Cystatin C＇left ventricular hypertrophy and heart function in elderly hypertensives. Methods According to the LVMI and the heart function grade of NYHA,110 elderly hypertensives were divided into left ventricular hypertrophy group（ LVH Group） and non-hypertrophic group（ non-LVH Group）,the relationship between each group＇s levels of serum Cystatin C and the cardiac function was studied. Results The levels of serum Cystatin C in the LVH group was significantly raised by contrast with that in the NVH group,there were statistically significant differences（ P〈0. 01）. The Cystatin C concentration of serum had positive correlation with LVDD,IVS,LVPW and LVMI（ r = 0. 27 - 0. 35,P〈0. 01）. Cystatin C had a negative corre lation with EF value（ r =-0. 39,P〈0. 01）,and a positive correlation with BNP level（ r = 0. 29,P〈0. 01）. The Cystatin C level had a positive correlation with the class of heart function（ r = 0. 56,P〈0. 01）. Conclusions The levels of serum Cystatin C had close relationship with the left ventricular hypertrophy and heart function in elderly hypertensives,and it may used as an important indicator of predicting the levels of left ventricular hypertrophy and heart function.
