HBV转归与机体体液免疫功能状况的相关性研究

摘　　要：目的 探讨乙型肝炎患者外周血体液免疫功能状况和血清IFN-γ、TNF-α变化与疾病转归关系.方法 采用散射比浊法检测352例乙肝病毒性肝炎患者血清IgG、IgA、IgM和C3、C4水平.结果 不同HBV-DNA载量、急性或亚急性肝衰竭组和HBV慢性感染组HBV患者血清IgG、IgA、IgM水平较正常对照组则升高（P＜0.05）,而C3、C4水平较正常对照组均降低（P＜0.05）;急性白限性HBV组和隐匿性HBV组血清IgG、IgA、IgM及C3、C4水平与正常对照组比较差异无统计学意义（P＞0.05）：HBc-IgM阳性患者经IFN-α治疗6个月后C3、C4水平有明显提高,血清IgG、IgA、IgM趋向正常,HBV-DNA阴转率达37.5％,与基础治疗组比较差异有统计学意义（P＜0.05）.结论 乙肝患者存在体液免疫功能紊乱,体液免疫功能状况与HBV的临床转归存在一定的相关性,其检测对乙肝的诊断、治疗及预后的判断有着一定临床意义.Objective To explore the correlation of the organism humoral immune function situation and the changes of serum IFN-γ and TNF-α with the clinical prognosis in HBV patients.Methods Scattering nephelometry was used to detect the serum levels of IgG,IgA,IgM,C3,and C4 in 352 HBV patients.The level sof serum IFN-γ and TNF-α were detected by ELISA.82 healthy subjects were chosen as controls.Results The levels of IgG,IgA and IgM of the patients with different DNA capacities,acute or subacute liver failure,or chronic HBV infection HBV were higher than those of the controls （P〈0.05） while the serum levels of C3 and C4 were lower than those of the controls （P〈0.05）.The levels of IgG,IgA,IgM,C3,and C4 of the patients with acute self-limited HBV or occult HBV were not statistically different from those of the controls （P〉0.05）.After IFN-α treatment for six months,the HBc-IgM positive HBV patients＆#39;C3 and C4 levels were significantly increased.IgG,IgA,and IgM levels were getting normal,and the HBV-DNA negative rate was 37.5%,with no statistical difference from basic treatment （P〈0.05）.Conclusions Patients with HBV have the disorder of humoral immunity.Humoral immune function situation correlates with the clinical prognosis of patients with HBV.Detecting Humoral immune function plays an important role in HBV patients＆#39; diagnosis,treatment,and prognosis.
