改良尿道扩张术治疗尿道球部损伤（附18例）

摘　　要：目的 探讨改良尿道扩张术治疗尿道球部损伤的价值.方法 将F6金属导芯置入F18导尿管管腔,前段弯曲如尿道扩张器,行尿道扩张并留置导尿.结果 18例尿道球部损伤患者经过常规留置导尿失败,改用本法,全部成功置入导尿管,无一例转手术治疗,2～3周拔除导尿管,随访半年至5年,无主诉排尿困难和勃起功能障碍.结论 尿道球部损伤大多数属于尿道挫伤,改良尿道扩张术治疗尿道球部损伤是一种有效、简便、微创的治疗方法.Objective To evaluate the clinical value of modified dilation of urethra in the treatment of bulbar urethral injury.Methods F6 wire guide was inserted into the lumen of urethral catheter （F 18）.The anterior extremity of wire guide was curved like a urethreurynter.Urethral sounding and urethral catheterization were performed by this modified urethral catheter.Results All the 18 patients were catheterized successfully after the failure of routine urethral catheterization and none was subjected to open surgery.All catheters were removed 2 to 3 weeks postoperatively.6-60 months＆#39; follow-up showed that none suffered from dysuresia and erectile dysfunction.Conclusions Most bulbar urethra disruption injuries are contused wound or partial rupture.Modified urethral catheter realignment,which has the advantages of short operation time,little pain,and minimal postoperative morbidity,is a convenient,minimal invasive and effective treatment for injured bulbar urethra.
