2型糖尿病患者血清β-淀粉样蛋白水平及其与胰岛素抵抗关系的研究

摘　　要：为探讨 2型糖尿病 (DM)患者 β-淀粉样蛋白 (β- AP)水平及其与胰岛素抵抗 (IR)的关系 ,了解 2型 DM与老年性痴呆 (AD)的内在联系 ,将 6 3例无认知障碍及脑血管病的 2型 DM患者按病程分为 DM1组 (2 2例 ,病程≥ 5年 )和 DM2组 (4 1例 ,病程 <5年 ) ,并与 2 9例健康者作对照 ,分别测定β- AP、空腹血糖 (FPG)、空腹胰岛素(FINS)、糖化血红蛋白 (Hb A1 C)、总胆固醇 (TC)及甘油三酯 (TG) ,计算胰岛素敏感指数 (ISI)。结果显示 ,DM组β- AP明显高于对照组 ;DM1组和 DM2组的各项指标均无明显差异 ;DM组β- AP与 FPG、FINS、Hb A1 C呈正相关(P分别 <0 .0 2 5、<0 .0 2 5和 <0 .0 5 ) ,与 ISI呈显著负相关 (P<0 .0 1) ,与 DM病程、TC、TG无明显相关性(P均 >0 .0 5 )。认为 2型 DM患者存在β- AP表达增加 ,IR可能起促发作用 ,2型 DM与 AD可能有一个共同的病理机制 ,IR参与了此机制。To probe the relation between serum level of β-amyloid protein(β-AP) and insulin resistance (IR) in patients with type-2 diabetes mellitus (DM),as well as to comprehend the inherent relation between type-2DM and Alzheimer,s diseases(AD).63 patients with type-2 DM free of cognition disorder and cerebrovascular diseases were classified into DM1 (≥5 years including 22 patients) and DM2 (<5 years including 41 patients)according to disease course, while 29 healthy people were referred as control group. Measured parameters were β-AP, fasting plasma glucose(FPG)'fasting insulin(FINS), glycated hemoglobin(HbA 1C),total cholesterol(TC) and triglyceride(TG).To calculate insulin sensitivity index(ISI). Result showed that serum level of β-AP in patients with DM was significantly higher than that of the control group. Parameters between DM1 and DM2 had indicated little difference. In patients with DM, the β-AP had a positive correlation with FPG( P<0.025),FINS( P<0.025)and HbA 1C( P<0.05),had a negative correlation with ISI( P<0.01),and had no obvious correlation with disease course( P>0.05), TC( P>0.05), TG( P>0.05).These suggest that patients with type-2 DM has a tendency of over-expression of β-AP, and IR may play a role of excitation .Type-2 DM may share a common pathological mechanism with AD and IR may participate in the mechanism.
