辽宁地区代谢病患病率的普查和癌症患者中的氨基酸代谢紊乱的研究

摘　　要：目的:通过采用串联质谱技术对3 363例辽宁地区成人患者进行代谢病检测,分析其代谢病的发病率和癌症患者中氨基酸代谢紊乱情况.方法:利用串联质谱技术,对辽宁地区的3 363例患者进行氨基酸、酰基肉碱及其相对应比值的检测,对代谢病患病率、各种疾病发生代谢病的概率和癌症患者中代谢病指标和预后进行分析.结果:采用串联质谱分析法测定3 363例成人患者的氨基酸和肉碱水平,确诊遗传代谢病病例1 033,其中男性612例、女性421例.阳性率是30.7%.其概率远远高于新生儿的发病率.肿瘤科室、消化科室和神经科室的患者阳性发病率较高,均高于30%.另外针对486例肿瘤患者中的299例乳腺癌患者进行分析,发现乳腺癌与谷氨酸代谢密切相关,谷氨酸代谢指标异常且降低.274例肺癌患者中组氨酸代谢较异常且降低.串联质谱方法对成人的代谢病检测对于患者早期筛查和诊断以及预后有重要的应用意义.结论:谷氨酸可作为乳腺癌患者早期诊断的一个特异性指标,同样肺癌中组氨酸的表现为低于正常下限,这个指标可以应用于肺癌的早期诊断中.这两个指标对于个体化治疗和预后也有很重要的应用价值.AIM：To analyze the incidence rate of metabolism and the amino acid metabolism disorders in cancer patients by de-tecting metabolic diseases in 3 363 adult patients in Liaoning area via the tandem mass spectrometry method.METHODS：In the present study,we used the tandem mass spectrometry method to i-dentify the index of amino acid,acylcarnitine and the ratio be-tween them,and analyze the prevalence of metabolic disease,in-cidence rate of metabolic disease in various diseases,and the met-abolic diseases incidence rate and prognosis of patients with canc-er.RESULTS：The index of amino acid and acylcarnitine of 3 363 patients were detected by tandem mass spectrometry meth-od.A total of 1 033 patients were diagnosed as inherited metabol-ic diaeases （male 612 cases and female 421 cases）,with the pos-itive rate being 30.7% and the incidence rate much higher than that of the neonatal.The positive rate in department of oncology, gastroenterology and neurology are higher than 30% averagely. An analysis of 299 patients with breast cancer showed that gluta-mate was closely related to the breast cancer,and glutamate me-tabolism index was abnormal and decreased.Likewise,the histi-dine metabolism was abnormal and reduced in 274 lung cancer pa-tients.Tandem mass spectrometry detection was of great impor-tance in early screening,diagnosis and prognosis of metabolic dis-eases in adults.CONCLUSION：Glutamate could serve as a specific indicator for early diagnosis of breast cancer.In lung cancer patients,the ammonia acid index is lower than normal in-ferior limit,which means that the ammonia acid index can be ap-plied in the early diagnosis of lung cancer.Glutamate and ammo-nia acid are significant for individualized treatment of cancer pa-tients and prognosis.
