摘 要:目的:探讨血浆脑源性神经营养因子(BDNF)与抑郁症合并2型糖尿病的相关性。方法选取我院2012年2月-2013年11月心境障碍科收入的抑郁症患者210例为研究对象,采用17项汉密尔顿抑郁量表(HAMD-17)评估患者抑郁严重程度,并测血糖。根据结果,将患者分为观察组(抑郁症合并2型糖尿病组)72例和对照1组(抑郁症非2型糖尿病组)138例,同时选取同期来我院进行体检的健康者70人为对照2组。运用ELISA 法对3组对象血清中的BDNF水平进行测定;采用PCR-RFLP法检测BDNF基因Val66Met的多态性;统计分析Val66Met的多态性与抑郁症的相关性。结果3组对象血浆中的BDNF水平的差异有统计学意义(F=18.73,P<0.05);观察组与对照1组HAMD评分差异有统计学意义(t=-3.423,P<0.01);3组间基因型分布(χ^2=9.48,P<0.05)和等位基因频率(χ^2=7.02,P<0.05)的差异均有统计学意义。结论抑郁症合并2型糖尿病的发生与血浆脑源性神经营养因子之间存在相关性,其中BDNF-Val66Met基因多态性在该病的发病机制中发挥重要作用。Objective To explore the correlation between plasma brain-derived neurotrophic factor (BDNF) and depression with type 2 diabetes mellitus .Methods 210 patients with depression who re‐ceived treatment in our hospital from February ,2012 to November ,2013 were taken as our research objects .Hamilton depression rating scale (HAMD)-17 was used to evaluate the severity of depression and measure the blood glucose of these patients .Based on their results ,we divided them into the observation group(depression with type 2 diabetes mellitus patients ,72) ,control 1 group (depression with non-type 2 diabetes mellitus patients ,138) Meanwhile ,we took 70 patients who had physical examination in our hospital and proved to be healthy in the same period as control 2 group .We adopted the ELISA method to measure the level of BDNF in the serum of the three groups ;and we adopted the PCR-RFLP method to test the BDNF (Val66Met) gene polymorphisms .We analyzed the correlation of BD‐NF (Val66Met) gene polymorphisms with depression .Results There were significant difference on the level of BDNF in the serum in three groups (F = 18 .73 ,P 〈 0 .05) .The HAMD scores of observation group were lower than group 1 (t = - 3 .423 ,P 〈 0 .01) .There exist statistical differences on genetic makeups of Val66Met in the three groups (P〈 0 .05) .The Val66Met gene polymorphisms had a significant correlation with depression (P 〈 0 .05) .Conclusions A strong correlation exists between BDNF and depression with type 2 diabetes mellitus .BDNF- Val66Met gene polymorphisms plays a key role in the pathogenesis of this disease .
