摘　　要：依托数字化医院优质医疗资源,将慢性病治疗和后期管理有效衔接,为患者提供持续、高质量的全程慢性病管理服务,将过去的被动治疗管理转变为主动的全方位管理,从而达到延缓慢性病进程、减少并发症、提高生活质量、降低医疗费用的目的。Relying on high quality medical resources of digital hospital, the treatment of chronic diseases and the later period management was effectively linking up. For the patients, we provide chronic disease management of consistent, whole service and high quality. To change the past passive management model to the full range of active management. The new management model of chronic disease can reduce chronic disease process, reduce the complications, improve the quality of life, reduce the cost of medicine.
