下尿路症状老年女性患者尿流动力学临床分析

摘　　要：目的:研究具有下尿路症状(LUTS)老年女性患者的尿流动力学异常改变及其临床意义。方法:给予171例有下尿路症状的老年女性患者尿流动力检查,分析相关参数并根据结果进行分类和治疗。结果:膀胱感觉过敏者76例,膀胱感觉减退者16例。膀胱容量缩小者56例,膀胱容量增大18例。高顺应膀胱14例,低顺应膀胱35例。膀胱过度活动15例,膀胱逼尿肌-盆底肌协调性紊乱11例。逼尿肌压力减退(或无张力)106例,逼尿肌压力增高12例。膀胱颈口梗阻6例。压力性尿失禁32例,Ⅰ型8例,Ⅱ型7例,Ⅲ型17例。急迫性尿失禁6例。充盈性尿失禁3例。检查结果全部正常者27例。结论:尿流动力学检查能准确评估老年女性LUTS患者的膀胱尿道功能状况,给予临床诊断治疗提供客观可靠的依据。Objective：To investigate urodynamic changes and clinical significance of elderly females with lower urinary tract symptoms（LUTS）. Method：171 elderly female patients with LUTS underwent urodynamic examinations,classifications and treatments were carried out according to the results analyse.Result：There were 76 cases of over-sensitive bladder,16 cases of low-sensitive bladder.56 cases of reduced bladder capacity,18 cases of increased bladder capacity.There were 14 cases of high compliance bladder,and 35 cases of low compliance.15 cases had overactive bladder,11 had detrusor/external sphincter dyssynergia.6 cases had bladder neck obstruction.106 cases of detrusor underactivity or areflexia,12 cases of detrusor pressure increased.Among 32 of patients with stress incontinence,8 cases were typeⅠ,7 cases were typeⅡ,17 cases were typeⅢ.There were 6 urgent incontinence cases and 3 overflow incontinence cases.27 cases were normal.Conclusion：Urodynamic study is useful in evaluating the status of bladder and urethra in elderly females with LUTS,and providing reliable evidences for clinical diagnoses and therapies.
