血浆病毒载量检测在肾综合征出血热诊断中的作用研究

摘　　要：目的 探讨血浆病毒载量检测在肾病综合症出血热诊断中的临床价值,并对疾病进展、血浆病毒载量和cGDNA水平的相关性进行分析,为该病诊断及治疗提供理论依据.方法 选取西安医学院第二附属医院门诊2012年2月-2014年2月收治的41例肾综合征出血热患者进行研究,同时选取同期健康体检者40例作为对照,通过实时定量RT-PCR的方法来检测研究对象血浆中病毒载量,采用荧光染色法检测标本中cGDNA水平,分析其与患者的血浆病毒载量之间的相关性.结果 疾病早期病毒载量和血浆cf-DNA均有升高,当病程进展到多尿期时又有所下降直至正常水平.疾病早期重型或危重型患者血浆病毒载量和cf-DNA水平高于轻型和中型患者（P＜0.05）,且患者血浆cf-DNA与血浆病毒载量呈正相关（r=0.76,P＜0.05）.结论 血浆cf-DNA水平和病毒载量可用来预测疾病进展及预后.Objective To study the relationship between plasma viral load and cf-DNA level in hemorrhagic fever cases（HFRS）,analyze the relationship among disease progress,plasma viral load and cfDNA levels,and to provide a theoretical basis for the diagnosis and treatment of HFRS.Methods 41 HFRS cases were included in the study in the second affiliated hospital of xi＆#39; an medical university from February 2011 to February 2014.40 people receiving health examination during the same period were included in the control group.Viral load in plasma samples was detected by RT-PCR method.cf-DNA level of the plasma samples was detected by fluorescence staining.The relationship between cf-DNA and plasma viral load was analyzed.Results The viral load and plasma cf-DNA level of test group were significantly higher than normal control subjects（P 〈 0.05）.These two indexes were elevated early in the disease,then dropped to normal level during he period of diuresis.These two indexes in severe and critical cases were higher than the mild cases （P 〈 0.05）.Correlation analysis showed that there was correlation between plasma viral load and cf-DNA （r =0.76,P 〈 0.05）.Conclusions Plasma viral load and plasma cf-DNA level can be used as predictors of disease progression and prognosis.
