切割球囊在糖尿病合并冠心病轻度钙化分支病变中的应用

摘　　要：目的评价切割球囊在糖尿病合并冠心病轻度钙化分支病变中应用的安全性及有效性。方法选择2012年7月至2014年1月在中国人民解放军第187医院及海南医学院附属医院收治的糖尿病合并冠心病轻度钙化分支病变并接受经皮冠状动脉介入(PCI)治疗的患者60例,随机等分为两组各30例,观察组采用切割球囊血管成形术(CBA),对照组采用普通球囊血管成形术(POBA),球囊扩张后如必要时植入支架,比较两组球囊扩张前的参考段血管径(RD)、最小血管径(MLD),以及扩张后的即刻获得、残余狭窄程度、血管夹层发生率。结果观察组扩张后即刻获得水平和残余狭窄程度明显优于对照组,差异有统计学意义(P<0.05);观察组的血管夹层发生率为3.33%(1/30),明显低于对照组的36.67%(11/30),差异有统计学意义(P<0.05)。结论切割球囊治疗糖尿病合并冠心病钙化分支病变效果优良,安全性高,值得推广。Objective To evaluate the efficacy and safety of cutting balloon in mild calcified branch lesions of diabetes combined with coronary heart disease （CHD）. Methods Sixty patients with mild calcified branch lesions in diabetic CHD treated with percutaneous coronary intervention （PCI） in the 187^th Hospital of Chinese PLA and the Affiliated Hospital of Hainan Medical University between July 2012 and January 2014 were enrolled in the study. The patients were randomly divided into two groups：the CBA group （observation group） and the POBA group （control group）. Stents were implanted after the balloon expansion if necessary. The RD and MLD before expansion, and the immediate acquire, residual stenosis degree and incidence of vascular dissection after expansion of the two groups were compared. Results The immediate acquire and residual stenosis degree of CBA group after expansion was significantly better than those of the POBA group, and the difference was statistically significant （P〈0.05）;The incidence of vascular dissection in CBA group was 3.33%, which was significantly lower than 36.67%of the POBA group. The difference was statistically significant （P〈0.05）. Conclusion The cutting balloon treatment in mild calcified branch lesions of diabetes combined with CHD has good effect, high safety, and worth promoting.
