云南省中医药治疗艾滋病患者脱失的分布情况及脱失原因分析

摘　　要：目的分析云南省2005-2013年接受中医药治疗HIV/AIDS患者脱失的分布情况及脱失原因,为提高患者依从性、进一步降低脱失率提供依据。方法通过国家中医药治疗艾滋病试点项目（云南）临床资料数据库,用描述流行病学分析方法,对HIV/AIDS患者脱失情况进行分析。结果云南省接受中医药治疗的HIV/AIDS患者脱失356例,脱失率为4.27%;脱失患者主要集中在20-49岁人群,占87.65%（291/332）;脱失患者中性传播和静脉吸毒感染途径分别占54.81%、34.94%;脱失患者以农民居多,占51.51%;脱失时间主要集中在治疗1年内,占68.26%（243/356）;脱失原因主要为自动退出（34.55%）、不愿服药（28.65%）、转西医治疗17.70%。结论云南省中医药治疗艾滋病患者脱失的主要原因是对中医药治疗的认知程度不高,需要加大宣传教育和随访力度,进一步提高患者依从性,降低脱失率。Objectives To explore the distribution and the reasons for drop out of of AIDS patients under Chinese traditional medicine treatment in Yunnan Province from 2005 to 2013,and to provide basis for improving adherence and reducing dropout rate.Methods The drop outs of AIDS under Chinese traditional medicine treatment were analyzed by descriptive epidemiological analysis methods through the National Pilot Project（Yunnan）clinical information databases.Results During the period,356 cases dropped out,accounting for 4.27% among the TCM intervention of HIV/AIDS patients in Yunnan.The patients were mainly concentrated in the 20 to 49years old,accounting for 87.65%（291/332）,with sexually transmitted routes of infection and intravenous drug infection accounting for 54.81% and 34.94% respectively.The dropouts were mostly farmers,accounting for 51.51%.The drop out time focused within one year of treatment,accounting for 68.26%（243/356）.The main reason for drop out were automatic withdrawal（34.55%）,unwilling to medication（28.65%）,turning to western medicine（17.70%）.Conclusions The main reason for drop out is that the awareness of Chinese traditional medicine treatment is not high.We should strengthen the education and follow-up efforts to further improve the patient compliance and reduce the dropout rate.
