老年人医疗服务利用及其影响因素分析——基于中国健康与养老追踪调查的数据

摘　　要：目的探讨老年人医疗服务利用情况,并分析其影响因素。方法基于中国健康与养老追踪调查(China Health and Retirement Longitudinal Study,CHARLS)的数据,采用描述性分析和Logistic回归分析,分析不同特征老年人门诊和住院利用程度及相关影响因素。结果老年人中,自评健康差、患有慢性病、遭受急性健康冲击将提高老年人门诊和住院利用率,而家庭年消费支出低、无医疗保险将限制医疗服务的利用。结论需要变量中的健康水平和控制变量中的经济水平、是否参保是老年人医疗服务利用的主要影响因素。应加强慢性病控制与管理,提高老年人健康水平,同时提高老年人医疗服务利用的经济可及性。Objectives To explore the utilization of health care and the related influencing factors in the elderly.Methods Based on CHARLS data,using descriptive analysis and logistic regression to analyze the utilization of both outpatient service and inpatient service and the influencing factors.Results Bad self-rated health,chronic diseases and acute health shock would increase both visiting rate and admission rate,while lower consumer spending and no insurance impeded the utilization.Conclusions Health status,as need variable,along with the control variables,the economic level and insurance type have significant effect on the utilization of health care.It＇s essential to enhance the control and management of chronic diseases,improve health status as well as increase the economic access to health utilization.
