温阳散瘀散外敷联合血栓通治疗下肢动脉硬化闭塞症随机平行对照研究

摘　　要：[目的]观察温阳散瘀散外敷联合血栓通治疗下肢动脉硬化闭塞症疗效。[方法]使用随机平行对照方法,将128例住院患者按病志号抽签方法简单随机分两组。对照组64例血栓通0.5g+0.9%氯化钠100m L,静滴,1次/d。治疗组64例温阳散瘀散(附子制、干姜各10g,细辛3g,桂枝、当归、延胡索、白芍各10g,鸡血藤15g,乳香、没药、川椒、红花、姜黄各10g,威灵仙15g,透骨草30g),1剂/d,2次/d,温度控制40℃,加热外敷;血栓通治疗同对照组。连续治疗2周为1疗程。观测临床症状、体征积分(皮温、肤色、发凉感、肢体酸胀、麻木、间歇性跛行、静息痛)、踝肱指数(ABI)、趾肱指数(TBI)、不良反应。治疗1疗程,判定疗效。[结果]治疗组显效21例,有效39例,无效4例,总有效率93.75%。对照组显效17例,有效35例,无效12例,总有效率81.25%。治疗组疗效优于对照组(P<0.05)。皮肤温度、静息痛积分两组均有明显降低(P<0.01),肢体麻木、酸胀治疗组有明显降低(P<0.05),对照组无明显差异(P>0.05),治疗组降低优于对照组(P<0.01)。ABI、TBI两组均有明显升高(P<0.01),治疗组升高优于对照组(P<0.01)。[结论]温阳散瘀散外敷联合血栓通治疗下肢动脉硬化闭塞症,疗效满意,无副作用,值得推广。[Objective] Efficacy of topical Wenyang scattered stasis combined Xueshuantong treatment of arteriosclerosis obliterans. [Method] Randomized parallel-group Method using the 128 cases of hospitalized patients by disease Zhi No. balloting simple random into two groups. 64 cases in the control group Xueshuantong 0.5g ＋ 0.9% sodium chloride 100 m L, infusion, 1 times / d. Treatment group 64 cases scattered Wenyang stasis（system aconite, ginger each 10 g, Asarum 3g, Gui, angelica, Corydalis, white peony root 10 g, Millettla 15 g, frankincense, myrrh, Sichuan pepper, saffron, turmeric each 10 g, Clematis 15 g, Speranskia 30g）, decoction 200 ml, morning and evening orally. Temperature control 40℃, heating topical; Xueshuantong treatment with the control group. 2 weeks of continuous treatment for a course of treatment. Observation of clinical symptoms and signs integral（skin temperature, skin color, hair cool feeling, body soreness, numbness, intermittent claudication, rest pain）, ankle-brachial index（ABI）, toe-brachial index（TBI）, adverse reactions. A course of treatment to determine efficacy. [Results] treatment group 21 cases, effective 39 cases, 4 cases ineffective, the total efficiency of 93.75%. 17 cases in the control group were cured, 35 cases effective, 12 cases ineffective, the total efficiency of 81.25%. Treatment group was better than the control group effect（P〈0.05）. Skin temperature, rest pain decreased significantly in both groups integral（P〈0.01）, numbness, soreness treatment group was significantly lower（P〈0.05）, no significant difference in the control group（P〈0.05）, treatment group lower priority in the control group（P〈0.01）. ABI, TBI groups were significantly higher（P〈0.01）, increased treatment group than the control group（P〈0.01）. [Conclusion] Wenyang stasis scattered topical therapy combined Xueshuantong arteriosclerosis obliterans, Results were satisfactory, no side effects, it is worth promoting.
