半夏泻心汤联合西药治疗慢性萎缩性胃炎随机平行对照研究

摘　　要：[目的]观察半夏泻心汤联合西药治疗慢性萎缩性胃炎疗效。[方法]使用随机平行对照方法,将100例住院患者按就诊顺序号法简单随机分为两组。对照组50例维酶素,0.8g/次,3次/d,饭前30min口服;胃复春,1.44g/次,3次/d,饭前30min口服。治疗组50例半夏泻心汤(黄连6g,大枣4颗,黄芪、半夏各15g,党参2g,三七粉分冲2g,乌贼骨、玄胡、白花蛇舌草各3g,丹参2g,三棱15g,甘草6g;痞满加枳实、砂仁;纳差加木香、鸡内金、砂仁;胃脾痛加白芍;大便干燥、数天不排便加决明子、麻仁;幽门螺旋杆菌阴性加蒲公英、鱼腥草;舌苔厚腻加白术、苍术、薏苡仁),1剂/d,水煎300m L,早晚服用;西药治疗同对照组。连续治疗90d为1疗程。观测临床症状、胃膜色泽、胃腺体萎缩、颗粒状增生、不良反应。治疗1疗程,判定疗效。[结果]临床疗效:治疗组显效13例,有效27例,无效10例,恶化0例,总有效率100.00%。对照组显效4例,有效15例,无效21例,恶化10例,总有效率38.88%。病理疗效:治疗组显效15例,有效30例,无效5例,恶化0例,总有效率90.00%。对照组显效8例,有效16例,无效16例,恶化10例,总有效率48.00%。治疗组疗效优于对照组(P<0.01)。[结论]半夏泻心汤联合西药治疗慢性萎缩性胃炎效果显著,无严重不良反应,值得推广。[Objective] To observe Banxiaxiexin Tang western medicine combined effect of chronic atrophic gastritis. [Methods]Using random parallel control method, 100 cases of hospitalized patients with treatment order number method is simple and randomized into two groups. Control group, 50 cases Weimeisu, 0.8g / time, 3 times / d, orally 30 min before meals; Weifuchun, 1.4g / time, 3 times / d, orally 30 min before meals. Treatment group 50 cases Banxiaxiexin Tang（Huanglian 6g, Dazao 4, Huangqi,Banxia 15 g, Dangshen 2g, Sanqifen 2g, Wuzeigu,Xuanhu,Baihuasheshecao 3g,Danshen 2g, Sanleng 15 g, Gancao 6g; fullness Zhishi,Sharen; anorexia plus Muxiang,Jineijin,Sharen; stomach and spleen pain plus Baishao; dry stools, plus a few days without a bowel Juemingzi,Maren; Helicobacter pylori-negative plus Pugongying,Yuxingcao; thick and greasy tongue plus Baizhu,Cangzhu,Yiyiren）, a / d, decoction 300 m L, sooner or later take; western medicine with the control group. 90 d is a continuous course of treatment. Observation of clinical symptoms, stomach membrane color, gastric glandular atrophy, granular hyperplasia, adverse reactions. A course of treatment to determine efficacy. [Results] Clinical efficacy： treatment group 13 cases, effective 27 cases, ineffective in 10 cases, worsening 0 cases, the total efficiency of 100.00%. Control group, four cases markedly effective in 15 cases, 21 cases, 10 cases of deterioration, the total efficiency of 38.88%. Pathological efficacy： treatment group 15 cases, effective 30 cases, ineffective five cases, worsening 0 cases, the total efficiency of 90.00%. Control group, 8 cases were markedly effective in 16 cases, 16 cases, 10 cases of deterioration, the total efficiency of 48.00%. Treatment group than the control group（P〈0.01）. [Conclusion] Banxiaxiexin Tang combined with Western treatment of chronic atrophic gastritis effect is significant, it is worth promoting.
