双水平气道正压通气联合茶碱治疗重叠综合征的临床观察

摘　　要：目的：探讨应用双水平气道正压通气（BiPAP）联合茶碱对重叠综合征（OS）患者的疗效。方法将105例 OS 患者，随机分为3组。A 组35例，在 BiPAP 治疗基础上，加用茶碱治疗；B 组35例，给予 BiPAP 治疗；C 组35例，给予吸氧治疗。观察并比较3组在疗效、动脉血气变化、多导睡眠图监测（PSG）、肺功能等方面的差异。结果治疗2个月后，3组患者二氧化碳分压（PaCO2）、呼吸暂停指数（AHI）、最长呼吸暂停时间（LAT）、Epworth 嗜睡评分量表（ESS）评分均降低，氧分压（PaO2）、氧饱和度（SaO2）、最低氧饱和度（LSpO2）、最高氧饱和度（HSpO2）、平均血氧饱和度（MSpO2）均升高，与治疗前比较差异均有统计学意义（P ＜0．05）。A 组与 B 组比较，LSpO2、HSpO2、MSpO2升高，AHI、ESS 评分降低，差异有统计学意义（P ＜0．05）。结论BiPAP 联合茶碱治疗重叠综合征比单纯 BiPAP 或氧疗治疗 OS 疗效更加明显。Objective To investigate the therapeutic effect of Bi-level Positive alrway Pressure （BiPAP）combined with aminophylline on overlap syndrome（OS）patients.Methods 105 patients were randomly divided into three groups.Group A（35 cases）were treated by BiPAP combined with aminophyl-line.Group B（35 cases）were treated only by BiPAP.Group C（35 cases）were treated only by oxygen therapy.The therapeutic effects,changes of oxygen content in arterial blood,polysomnography,pulmonary function were observed and compared between the two groups after therapy.Results After 2 months of treatment,there was a significant difference in arterial blood gas analysis,PSG and pulmonary function be-tween 3 groups.Group A and group B compared with group C,PaCO2 ,AHI,LAT,ESS were reduced, PaO2 ,SaO2 ,LSpO2 ,HSpO2 ,MSpO2 were increased.The differences have statistical significance （P 〈0.05）.Group A compared with group B,LSpO2 ,HSpO2 ,MSpO2 increased,AHI,ESS were reduced.The differences have statistical significance（P 〈0.05）.Conclusion The study shows that BiPAP combined with aminophylline therapy is more remarkably effective than BiPAP therapy or oxygen therapy.BiPAP combined with aminophylline therapy can significantly influence therapeutic efficacy in OS patients.
