摘 要:目的探讨创面分泌物培养前涂片镜检对判断培养结果的意义。方法对150份创面分泌物标本作涂片革兰染色镜检,同时进行细菌培养。根据涂片结果将标本分为Ⅰ、Ⅱ和Ⅲ三类,并将涂片结果与培养结果进行比较分析。结果涂片合格标本(含Ⅰ和Ⅱ类)90例,涂片不合格标本(Ⅲ类)60例,标本合格率为60%;Ⅰ、Ⅱ和Ⅲ类标本培养细菌检出率分别为97.4%、40.4%和38.3%,合格标本的细菌总检出率(64.4%)显著高于不合格标本的检出率(P<0.05);Ⅰ、Ⅱ类标本涂片结果与培养结果的符合率分别为97.4%、65.4%,合格标本的总符合率为78.9%。Objective To investigate the role of smear microscopic examination before bacterial culture in the estimation of culture outcomes.Methods A total of 150 specimens from wound secretion were performed gram staining microscopy and bacterial culture.All the specimens were classified into three categories,i.e.category Ⅰ,Ⅱ and Ⅲ according to the results of smears,and then the results of smears were compared with that of culture.Results A total of 90 smear specimens were qualified (including category Ⅰ and Ⅱ) and 60 specimens were unqualified (category Ⅲ),with a qualified rate of 60%.The positive rates of category Ⅰ,Ⅱ and Ⅲ culture specimens were 97.4%,40.4% and 38.3%,respectively.The total detected rate of qualified specimens was 64.4%,significantly higher than that of the unqualified specimens (P < 0.05).The consistency rates of category Ⅰ and Ⅱ specimens between smears and culture were 97.4% and 65.4%,and the total coincidence rate of qualified specimens was 78.9%.Conclusion Gram staining microscopic examination of wound secretion is valuable in improving the quality of specimens and the accuracy of pathogen culture.
