A_2B亚型输血相容性检测及输血方案选择

摘　　要：目的探讨A2B亚型输血相容性检测结果及其配合型输血方案选择。方法运用三步法分析血型鉴定正反定型不符的血样本。在不同试验条件下,进行ABO血型鉴定相关血型血清学实验;盐水法、微柱凝胶抗人球蛋白介质法进行交叉配血试验。结果发现1例患者血型血清学试验反应格局符合A2B亚型,同时含有高效价Ig M抗-A1抗体。采用O型洗涤红细胞配合型输血,未发生输血不良反应,输血疗效良好。结论 ABO血型鉴定正反定型不符时,应考虑有可能为ABO亚型;A2B亚型可伴有Ig M抗-A1抗体产生;不规则抗体筛查试验所用谱细胞为O细胞制备,不含有A、B抗原,不能检出ABO亚型中含有的不规则抗-A、抗-B抗体;A2B亚型患者选择配合型输血,输血疗效良好。Objective To report a example of subgroups of A2B about the transfusion compatibility testing results and the cooperate blood transfusion select. Methods Analyze the sample inconformity with forward typing and reverse typing of blood grouping with three-step process. Do experiments of blood group serologic investigation at different condition; Do cross matching test with NS method and Anti globulin medium micro column gel method. Results The blood group serological investigations results of one patient＇s are correspond with subgroups of A2B and this patient＇s blood is also contains high potency IgM antibody of anti-A~. Transfusion with washed RBC of group O, no adverse reaction was observed, curative effect is good. Conclusion We should consider the subgroups of ABO when it is inconformity with forward typing and reverse typing of blood grouping, subgroups of A2B may produce IgM antibody of anti-A~. The panel cell in the unexpected antibodies test are extraction by O cell, so it doesn＇t have A antigen and B antigen, and couldn＇t find the unexpected anti-A and anti-B of ABO blood group system. Patients of subgroups of A2B can choose cooperate blood transfusion which has excellent curative effective.
