蜡疗法在PICC置管并发送管困难病人中的应用

摘　　要：[目的]探讨蜡疗法对处理PICC置管时并发送管困难的效果。[方法]2013年8月—2014年2月,我院首次化疗并行PICC置管的病人共560例,其中并发送管困难病人80例,采用随机数字表法分为蜡疗组和热水袋组各40例。蜡疗组采用蜡疗法处理送管困难,热水袋组采用热水袋法处理。比较两组病人B超下不同时间点的血管直径、疼痛评分、穿刺次数、置管完成时间及静脉炎发生率的差异。[结果]我院PICC置管并发送管困难的发生率为14.29%。除干预后1min的血管直径差异无统计学意义之外,两组病人其余时间点的血管直径、疼痛评分差异均有统计学意义(P<0.05);穿刺次数、置管完成时间及静脉炎发生率差异亦均有统计学意义(P<0.05)。[结论]相比热水袋法,蜡疗法更能缓解血管痉挛、减轻疼痛、减少穿刺次数、缩短置管完成时间及降低静脉炎发生率,增加病人的舒适度。Objective：To probe into the effect of wax therapy for the treatment of pipe difficult in PICC patients . Methods：A total of 560 cases of patients received the first chemotherapy and PICC were selected in our hospital from 2013 August to 2014 February ,including 80 cases of patients complicated with pipe difficult .They were randomly divided into wax therapy group and hot water bag group by using the random number table method , 40 cases in each .The patients in wax therapy group received wax therapy for treatment of pipe difficult ,the pa‐tients in hot water bag group used hot water bag method for treatment ,then to compare the vascular diameter in B ultrasound ,pain score ,number of puncture ,catheter completion time and the incidence of phlebitis at differ‐ent time points between both groups .Results：The incidence of pipe difficult in our hospital was 14 .29% .In ad‐dition to vascular diameter at 1 min after intervention ,there was statistically significant difference in the vascu‐lar diameter ,pain score ,number of puncture ,catheter completion time and incidence of phlebitis in the remai‐ning time points between both groups（P〈0 .05） .Conclusion：Compared with hot water bag method ,the wax therapy can relieve vascular spasm ,reduce pain and the number of puncture ,shorten catheter completion time , reduce the incidence of phlebitis and increase patients’ comfort .
