患方视角下北京市某区乡镇卫生院医患信任状况研究

摘　　要：目的:从患方视角了解北京市乡镇卫生院的医患信任情况及其影响因素,为构建农村和谐的医患关系提供科学依据。方法:2014年5月5-6日对北京市H区14个乡镇卫生院的70名患者进行问卷调查,并进行统计学分析。结果:患方视角下,认为医生总体上信任患者的有61人(87.14%),认为患者总体上信任医生的有63人(90.00%)。认为医患信任建立的原因依次为医疗技术(88.57%)、道德品质(81.42%)、彼此熟悉(47.14%)。医生评价中,患者认可度低的依次为正确诊断(81.43%)、诊疗效果(81.43%)、不必要检查(82.86%)。医疗机构评价中,患者认可度低的依次为药品种类(60.00%)、诊疗设备(64.29%)、药品价格(71.43%)。卫生政策评价中,患者对"新农合"报销制度的知晓率最高(91.43%),且多数患者认为相关卫生政策对医患信任有提高作用。结论:今后要重视基层医疗卫生机构的发展,以患者的需求与医务人员的利益为导向制定相关卫生政策。同时加强乡镇卫生院卫生人力资源的建设,提高医务人员技术水平及工作积极性,建立和谐的医患关系。Objective： To investigate the doctor-patient trust status of township hospitals and its influencing factors in Beijing , so as to pro-vide evidence for harmonious doctor-patient relationship.Methods：We investigated and analyzed 70 patients in 14 township hospitals in H dis-trict of Beijing.Results：From the view of patients＇ side, 61 patients （87.14%） chose doctors generally trust patient and 63 patients （90.00%） chose patients generally trust doctors.In the type of doctor-patient trust, 88.57% patients chose medical skill, 81.42% patients chose medical ethics, 47.14% patients chose familiar with each other.The low approval degree of doctor was accurate diagnosis （81.43%）, treatment outcome （81.43%） and excessive imaging （82.86%）.The low approval degree of township hospitals were drug categories （60.00%）, medical equip-ment （64.29%） and drug price （71.43%）.The awareness percentage of New Rural Cooperative Medical System was 91.43%and most patients thought health policies improved doctor-patient trust.Conclusion：In order to improve doctor-patient relationship of township hospitals, it＇ s ur-gent to pay attention to developing basic medical units so as to make health policies based on both patients＇ need and doctors＇ interest and strength-en the building of human resources so as to improve medical skill and working initiative .
