摘　　要：目的 探讨抗甲状腺球蛋白抗体（TGA）检测对分化型甲状腺癌（DTC）的诊断价值.方法 回顾性分析上海市杨浦区五角场街道社区卫生服务中心2000年1月至2013年12月经超声诊断的甲状腺结节的患者,均常规检测TGA,筛选出在同一所上级医院手术治疗后来在本社区随访患者共98例（不包括合并桥本甲状腺炎）,依据病理检查结果分为恶性组（35例,为DTC）、良性组（63例,为结节性甲状腺肿或甲状腺腺瘤）.比较2组患者TGA水平及TGA阳性率.结果 恶性组患者TGA阳性率及TGA水平明显高于良性组[37.1％ （13/35）比11.1％ （7/63）、21.7 （9.6,126.8） kU/L比14.6（8.1,25.0） kU/L],差异有统计学意义（P＜0.05）.结论 在社区医院中TGA的检测对于DTC的早期诊断具有一定价值.Objective To observe the value of anti-thyroglobulin antibody（TGA） detection in the diagnosis of differentiated thyroid cancer （DTC） at community hospital.Methods Totally 98 followed-up patients with thyroid nodule who were diagnosed by ultrasound from January 2000 to December 2013 and subsequently received operation in the superior hospital were enrolled.According to the results of pathology,the patients were divided into malignant group （35 cases of DTC） and benign group [63 cases of nodular goiter （NG） or thyroid adenoma （TA）].The level of TGA level and positive rate of TGA were assessd and compared between the two groups.Results The TGA level and positive rate of TGA in malignant group were significantly higher than those in benign group [21.7 （9.6,126.8） kU/L to 14.6 （8.1,25.0）kU/L,37.1％ （13/35） to 11.1％ （7/63）] （P＜ 0.05）.Conclusion Anti-TgAb detection is useful for early diagnosis of DTC at community hospital.
