贵州省抽样调查点结核病人中HIV感染状况分析

摘　　要：目的通过随机抽样调查点结核病患者的HIV/AIDS筛查,分析贵州省TB/HIV双重感染现状。方法按照中国疾病预防控制中心结核病控制中心《结核病患者艾滋病病毒感染状况调查实施细则》对4个抽样调查点进行调查,对2012年4月1日-7月31日和2013年4月1日-7月31日在调查点诊断为活动性结核患者开展HIV筛查。结果在4个抽样调查点的1 006例活动性结核病患者中新发肺结核和肺结核合并肺外结核病患者930例,占所有结核病患者的92.45%,结核性胸膜炎和复发肺结核占7.55%;结核病患者中筛出2例HIV阳性,双重感染率为0.20%;2例HIV阳性者同县不同乡,该县结核病患者中的HIV阳性感染率为0.60%。两例TB/HIV双重感染者男女各1例,1例46岁、1例48岁,均为农民,为涂阳肺结核病患者,感染HIV的途径为异性传播,既往未做过HIV的筛查。结论贵州省调查点的HIV感染处于较低水平,但对于HIV较高感染的县需密切关注疫情变化,并治疗TB/HIV双重感染患者。Objective To analyze double infection status of TB/HIV through investigation of random samphng counties in Guizhou province. Methods Total of 1006 active tuberculosis patients were accepted HIV/AIDS screening among the TB patients in the 4 investigation counties in Guizhou province according to the Imple- mentation scheme of HIV/AIDS infection status among the TB patientstluring the 1st April to 31th July in 2012 and 2013. Results Among the 4 random sampling counties, 930 TB cases was new registered active pulmona- ry tuberculosis （PTB） or PTB amalgamated extra pulmonary tuberculosis, accounted for 92. 45% （930/ 1006） . Tuberculosis pleuritis and retreatment of pulmonary tuberculosis was accounted for 7.55% （76/ 1006） . There were 2 HIV positive among TB patients and TB/HIV double infection rate was 0. 20% （2/ 1006） in 4 investigation counties. 2 HIV positive patients lived in the same county but not in the same town- ship, and the infectious rate of HIV positive was 0. 60% in this county. In 2 TB/HIV patients, one female was 48 years old and one male was 46 years old. both were peasants and new smear positive of PTB. The 2 TB/HIV patients were heterosexual transmission and they were not screened HIV in their past medical history. Conclusions The positive infectious rate of HIV is lower in 4 investigation counties in Guizhou province, But it is need highly pay attention to the epidemic change in the counties of positive infectious rate HIV, and take measure to treat patients of TB/HIV double infection.
