日常天灸结合集群免疫治疗常年性变应性鼻炎的临床观察

摘　　要：【目的】探讨提高变应性鼻炎集群免疫治疗效果的方法。【方法】将60例常年性变应性鼻炎患者随机分为治疗组和对照组各30例。治疗组给予集群免疫治疗结合日常天灸治疗,对照组单纯给予集群免疫治疗。观察2组治疗前后中医体质证候积分和生活质量评分的变化情况,并于治疗1年后进行临床疗效评定和安全性评估。【结果】(1)治疗1年后,治疗组总有效率为96.67%,对照组为80.00%,治疗组疗效优于对照组(P<0.05)。(2)治疗后,治疗组气虚、阳虚、特禀3种中医体质类型的证候积分均显著降低(P<0.01),治疗组在气虚、阳虚中医体质证候积分的改善方面显著优于对照组(P<0.05)。(3)治疗后,2组生活质量量表(RQLQ)各维度评分和总分均有不同程度改善,与治疗前比较,除情感问题外,差异均有统计学意义(P<0.01);治疗后组间比较,差异均无统计学意义(P>0.05)。(4)安全性方面,免疫治疗中2例出现1级全身不良反应,5例出现局部不良反应;天灸治疗过程中3例出现皮肤起水泡,经对症处理后症状消除。【结论】日常天灸结合集群免疫治疗对常年性变应性鼻炎具有较好临床疗效,能明显改善患者气虚、阳虚及特禀等中医体质证候,提高患者生活质量。Objective To explore a more effective way to enhance the clinical efficacy of cluster immunotherapy for perennial allergic rhinitis. Methods A total of 60 perennial allergic rhinitis patients were evenly randomized into treatment goup and control group. The two groups were given cluster immunotherapy, and the treatment group was given daily vesiculating moxibustion additionally . Before and after treatment, the scores of traditional Chinese medical constitution and the rhinitis quality of life （ QOL） were observed, and one year after treatment, the therapeutic effect and safety were evaluated. Results （ 1） One year after treatment, the treatment group showed better total clinical efficiency than the control group （ 96.67% vs 80.00%, P〈0.05）. （ 4） Two cases had grade 1 general adverse reaction and 5 cases had local adverse reaction during cluster immunotherapy. During vesiculating moxibustion, 3 cases had blistering and the blistering disappeared after symptomatic treatment. Conclusion Daily vesiculating moxibustion combined with cluster immunity therapy is effective for the treatment of allergic rhinitis, showing good effect on improving Qi deficiency constitution, yang deficiency constitution and special constitution as well as the quality of life of the patients.
