太仓市2009～2013年新涂阳肺结核患者发现与转归情况

摘　　要：目的分析太仓市新涂阳肺结核疫情的流行特点和规律,为探讨防制对策与措施提供科学依据。方法收集太仓市2009~2013年登记的新涂阳肺结核患者资料进行统计分析。结果太仓市2009~2013年共报告新涂阳肺结核患者660例,平均登记率14.17/10万,流动人口明显高于户籍人口;3~8月和10月病例数较多,占66.52%（439/660）;男性患者明显高于女性,在户籍人口中更为明显;户籍人口中患者以老年人为主,但女患者在25~34岁发病有个高峰,流动人口以青壮年为主,新涂阳病人治愈率为91.36%（603/660）,年龄和性别对治愈率有明显影响（P〈0.01）,户籍、患者来源、发病季节对治愈率影响不明显（P〉0.05）;治疗失败、死亡、丢失是导致患者未治愈的主要原因。结论太仓市应开展有针对性的防控措施来提高患者的发现和治愈率。Objective To analyze the epidemiological characteristics and regularity of new smear- positive pulmonarytuberculosis cases in Taicang city, and to provide scientific basis for control strategies. Methods Data of smear-positivepulmonary tuberculosis cases were collected and statistically analyzed. Results A total of 660 new smear-positive pulmonarytuberculosis cases were reported in Taicang city during 2009- 2013, with an average registration rate of 14.17/100 000,significantly higher in floating population than that in registered population. Most of the cases distributed in March-Augustand October,accounting for 66.52% of all cases（439/660）.The proportion of male patients was higher than that of femalepatients,especially in the census registered population. Among registered population most of the pulmonary tuberculosis caseswere senile cases and most cases among floting population were adults.There was an epidemic peak in age group of 25-34 yearsin women among the registered population. The cure rate of pulmonary tuberculosis was 91.36%（603/660）,significantlyinfluenced by age and gender factors（P〈0.01）. No significant influences of other factors including household registration, originof patients,onset season on cure rate were noticed（P〉0.05）. Treatment failure, death and default were the main causes for failureof cure. Conclusion Targeted prevention and control measures should be carried out to improve the case-finding and curerate.
