软镜与经皮肾镜在复杂性肾结石碎石术中的效果对比研究

摘　　要：目的比较软镜与经皮肾镜在复杂性肾结石碎石术中的效果。方法研究对象为2011年3月—2013年6月就诊的复杂性肾结石患者70例,根据随机数字表法将患者分为A组、B组,各35例。A组给予软镜碎石术,B组给予经皮肾镜碎石术。两组均在术后2、30、90、180 d复查腹部平片、超声。对比两组手术时间、住院时间、结石排净时间、一次结石清除率、并发症发生率。计量资料采用t检验,计数资料采用χ2检验,P<0.05为差异有统计学意义。结果 A组手术时间、住院时间、结石排净时间[(57.91±11.60)min、(3.11±1.29)d、(4.21±3.09)周]与B组[(46.30±10.84)min、(4.96±1.67)d、(7.79±4.08)周]比较,差异均有统计学意义(均P<0.05)。A组一次结石清除率、并发症发生率(94.29%、11.43%)与B组(77.14%、65.71%)比较差异均有统计学意义(均P<0.05)。结论软镜碎石术治疗复杂性肾结石与经皮肾镜比较效果好,并发症少,可以在临床广泛推广。Objective To compare the effect of between soft lens and percutaneous nephroscope in the lithotripsy of com- plexity kidney stones. Methods 70 cases of patients from March 2011 to June 2013 with the complexity of our hospital, according to random number table method divided the patients into group A and B,35 cases each. Group A was given soft lens lithotripsy, and group B with percutaneous nephrolithotripsy. All subjects were reexaminated with plain film Of the ab- domen and ultrasonography by postoperation of 2,30,90,180 days. Comparing the two groups of operative time,hospitali- zation time, the lithagogue time, astone clearance rate, complication rate. Count data was processed by t - test, Measurement data by chi - square test, P 〈 0.05 was considered statistically significant. Results The operation time, hospitalization time,stone net time of Group A were[ （57.91±11.60）min and（3.11±1.29）d,（4.21±3.09）week] and group B[ （46. 30±10.84） min and （4.96±1.67 ） d, （7.79±4.08） weeks ] were statistically significant differences （ P 〈 0.05 ）. Stone clearance rate,complication rate of group A were 94. 29%, 11.43% and group B were 77. 14%, 65.71%, differences were statistically significant（ P 〈 0.05 ）. Conclusions Compared with peroutaneous nephroscope,the effect of complexity kidney stones with soft lens lithotripsy is better,fewer complications,and this surgery way can be widely applied in clinic.
