重组人生长激素配合早期胃肠内营养在慢性阻塞性肺疾病急性加重期中的应用

摘　　要：目的探讨重组人生长激素对慢性阻塞性肺疾病急性加重期并呼吸衰竭机械通气患者的应用。方法采用临床随机对照试验,选取本院慢性阻塞性肺疾病急性加重期患者60例,随机分为对照组和治疗组,2组均配合早期胃肠内营养支持,并进行抗感染等综合治疗,治疗组加用重组人生长激素,以外周血CD4、CD8、CD3(T细胞亚群)活性作为研究内容,均于治疗前和治疗第7天、第10天清晨采集患者空腹静脉血检测血清总蛋白、白蛋白、免疫球蛋白、肝肾功能、血糖和血常规作为评价指标。比较2组的营养指标、机械通气时间和住ICU时间。结果治疗组患者用药7 d后血清总蛋白、白蛋白、免疫球蛋白和T细胞亚群活性均明显高于对照组(P<0.05)。结论短期使用重组人生长激素配合早期胃肠内营养支持,可望能缩短慢性阻塞性肺疾病急性加重期的机械通气时间,改善患者营养状况,增强免疫功能,提高临床疗效。OBJECTIVE To investigate the application of recombinant human growth hormone（ rh GH） on acute exacerbation of chronic obstructive pulmonary disease（ AECOPD） patients with mechanical ventilation. METHODS With the clinical randomized controlled trials,60 chosen AECOPD patients in the hospital were randomly divided into 2 groups,which were control group and treatment group. All patients were given therapies as early enteral nutrition support and antibiotics,while patients in the treatment group with recombinant human growth hormone（ rh GH） as well. The activity of blood CD4,CD8,and CD3（ T cell subset） was observed.Fasting blood samples were collected to test the total plasma protein,albumin,immune globulin,renal function,blood glucose and blood routine while before therapy,the 7th day and the 10 th day under treatment,so as to compare the nutrition indexes,duration of mechanical ventilation and ICU stays. RESULTS On the 7th day the serum levels of the total plasma protein,albumin,immune globulin and the activity of T cell of rh GH group under treatment significantly increased（ P〈 0. 05）. CONCLUSION Recombinant human growth hormone（ rh GH） with early enteral nutrition support would not only help shortening the duration of ventilatory support,but also improving nutrition condition and building up resistance of AECOPD patients.
