冠心病患者瘦素脂联素瘦素／脂联素及其影响因素分析

摘　　要：目的：分析冠心病患者瘦素、脂联素、瘦素／脂联素及其影响因素。方法选取91例冠状动脉造影确诊冠心病患者，静脉取血测定患者的总胆固醇（ TC ）、低密度脂蛋白胆固醇（LDL－C）、高密度脂蛋白胆固醇（HDL－C）、甘油三酯（TG）、C－反应蛋白（CRP）、白细胞介素－6（IL－6）、尿酸（UA）、血清瘦素及脂联素的浓度，并计算瘦素／脂联素的比值，行冠脉造影计算冠脉积分。结果①体质量指数（ BMI）与TC、TG、LDL－C、极低密度脂蛋白（ VLDL－C）、UA均有相关性；②脂联素与VLDL－C、CRP、IL－6有相关性，其中脂联素与VLDL－C呈负相关，与CRP、IL－6呈正相关，脂联素与CRP相关性相对较强（P＜0．05）；③瘦素／脂联素与CRP有相关性，高血压、糖尿病、瘦素／脂联素和CRP为危险因素，而HDL－C为保护因素（ P＜0．05）。结论 BMI与血脂高度相关，脂联素、瘦素／脂联素为冠心病的危险因素，且与CRP相关。Objective To analyze leptin, adiponectin, leptin/adiponectin and their influence factors in the patients with coronary heart disease （CHD）.Methods The present study included 91 CHD patients diagnosed by coronary angiography.Venous blood was sampled from these patients to determine total cholesterol （ TC ） , low density lipoprotein cholesterol （ LDL -C ） , high -density lipoprotein cholesterol （HDL-C）, triglycerides （TG）, C-reactive protein （CRP）, interleukin-6 （ IL-6） , uric acid （ UA） , the concentration of leptin, adiponectin, and leptin/adiponectin ratio; the coronary severity score was calculated after coronary angiography.Results ①Body mass index （ BMI） was related to TC, TG, LDL-C, VLDL-C and UA.②Adiponectin was related to VLDL-C, CRP and IL-6;adiponectin was negatively correlated with VLDL-C, but positively correlated with CRP, IL-6;the strongest correlation was shown between adiponectin and CRP （ P〈0.05 ） .③High blood pressure, diabetes, leptin/adiponectin, CRP were risk factors for coronary heart disease, but HDL-C was a protective factor for coronary heart disease （ P 〈0.05 ）; among these risk factors, leptin/adiponectin ratio and CRP had a correlation.Conclusion BMI and blood lipid are highly relevant. Leptin/adiponectin ratio is one of risk factors for coronary heart disease （CHD）, and it is associated with CRP.
