摘  要:目的:研究乌司他丁( ulinastatin, UTI)对50%总体表面积烫伤大鼠心肌的保护作用及其与保护性蛋白表达的关系。方法40只雄性SD大鼠随机分为烫伤组( n=20)和乌司他丁组( n=20),两组动物均造成50%总体表面积Ⅲ度烫伤,烫伤后即刻静脉分别注射生理盐水和乌司他丁40000 U/kg。于烫伤后6 h和24 h取大鼠腹主动脉血,测定血浆肌酸激酶同工酶( CK-MB)活性,采用干/湿质量法测定心肌组织含水量,用酶联免疫吸附法( ELISA)检测心肌组织热休克蛋白70(HSP70)的含量,用蛋白质免疫印迹法(Western blot)测定心肌细胞中乙酰化组蛋白H3赖氨酸9(Ac-H3K9)表达。结果烫伤后6 h乌司他丁组CK-MB和心肌组织含水量分别为[(1638.3±339.2)U/L和(68.14±1.93)%],显著低于烫伤组[(2271.8±350.9)U/L和(83.02±2.12)%](P<0.05);伤后24 h上述指标两组比较差异无统计学意义(P>0.05)。伤后6 h乌司他丁组心肌HSP70和Ac-H3K9表达水平均显著高于烫伤组(P<0.05或P<0.01)。伤后24 h上述指标两组比较差异无统计学意义(P>0.05)。结论乌司他丁能减轻致死性烫伤大鼠早期的心肌损害,减轻组织水肿,其机制可能与乌司他丁能增加大鼠心肌保护性蛋白表达,提高对烧伤的耐受力有关。Objective To investigate the protective effects of ulinastatin on myocardium in rats following 50%total body surface area burn injury and its relationship with the expression of protective protein.Methods Forty male Sprague -Dawley rats were randomly divided into scald group and ulinastatin group.Rats in the two groups were subjected to 50% total body surface area third-degree burns.Immediately after scald rats were intravenously given either 1 mL saline in scald group or 1 mL saline containing ulinastatin ( 40 000 U/kg ) in ulinastatin group.Blood of rats in each group was withdrawn from the abdominal aorta at 6 hours and 24 hours after injury for measurement of plasma creatine kinase-MB ( CK-MB) activities;the rates of tissue water content in heart were determined by the dry/wet weight.Heart tissues were harvested for the measurement of acetylated histone H3 by Western Blot and HSP70 by ELISA.Results The CK-MB and the water content of myocardial tissue after 6 h injury in ulinastatin group were (1638.3 ±339.2)U/L and (68.14 ±1.93)%, respectively, which were significantly lower than those in scald group [(2271.8 ±350.9)U/L, (83.02 ±2.12)%, respectively;P〈0.05)], but the changes in measurement had no significant difference after 24 h injury between the two groups (P〉0.05).The expression of histone H3 and heat shock protein 70 (HSP70) after 6 h injury in ulinastatin group were obviously higher than those in scald group ( P〈0.05 or P〈0.01), but the indexes had no significant difference after 24 h injury between the two groups ( P〉0.05).Conclusion The results indicate that ulinastatin significantly alleviate myocardial injury and inhibit tissue edema, reduce in early stage of burn injury.These protective effects may be due to its increasing expressions of histone H3 and HSP70 may be related to improvement of the tolerance heart to burn injury.
