老年颅脑损伤的临床特点及救治体会

摘　　要：目的 探讨老年颅脑损伤患者有效的救治方法.方法 随机选取老年颅脑损伤患者58例,分析患者的临床表现、病因、临床诊断及救治资料,随访患者治疗转归及预后情况.结果 本研究58例患者经手术治疗24例,非手术保守治疗34例;死亡4例（6.9％）,存活54例（93.1％）,其中康复效果较好40例（68.9％）,残疾13例（中度10例,重度3例）,植物生存1例.消化系统、呼吸系统、脑干伤、严重脑挫裂伤等重要器官并发症是导致患者死亡的主要原因.结论 老年颅脑损伤患者病情较为复杂,临床表现多样,极易并发各种全身并发症,应及早进行CT检查诊断,严格掌握手术指征,及时采取正确的治疗方法,降低患者的病残率和病死率,提高患者生存质量.Objective To investigate the effective treatment method for the elderly patients with craniocerebral injury.Methods Fifty-eight elder patients with craniocerebral injury were randomly selected as the research objects,the clinical presentation,etiology,clinical diagnosis and treatment data were analyzed,and the treatment outcome and prognosis were followed up.Results Among the 58 patients,24 cases received surgical treatment,34 cases received conservative nonsurgical treatment.Four cases died（6.9%）,54 cases（93.1%） survived,40 cases had good rehabilitation effect,deformed in 13 cases（10 cases of moderate and severe in 3 cases）,plant survival in 1 case.The important organs complications such as digestive system,respiratory system,the brain stem injury,severe cerebral contusion were the main causes of death.Conclusions The condition of elderly patients with craniocerebral injury is relatively complex,clinical manifestations are diverse,is easily complicated with a variety of systemic complications,CT examination should be made as early as possible,the surgical indications should be strictly grasped,adopt the right treatment in time,reduce the sick rate of patients,mortality rate,and improve patients quality of life.
