摘 要:目的通过Meta分析方法评估亚洲成人社会经济状况(SES)与代谢综合征(MS)的关系。方法两名研究者独立检索Pubmed、Embase、Sinomed、CNKI和万方数据库,时间截止到2014年9月16日,收集探讨亚洲地区成人SES的三个衡量指标(受教育水平、收入水平和职业类型)与MS关系的流行病学研究。对纳入的文献进行数据提取,应用Statal 2.0统计软件进行Meta分析,并进行异质性检验、发表偏倚分析和敏感性分析。结果共纳入相关的研究22篇,共4231158人。Meta分析结果显示,在总人群中,与受教育水平低者相比,中等和高等受教育水平者罹患MS的OR(95%CI)分别为0.82(O.76~0.90)和0.66(0.56~0.78);而收入水平和职业类型与MS的相关性无统计学意义。亚组分析后,在男性中,与低收入者相比,中、高收入水平者罹患MS的OR(95%CI)分别为1.30(0.98~1.74)和1.44(1.15~1.81)。在女性中,与受教育水平低者相比,中等和高等受教育水平者罹患MS的OR(95%CI)分别为0.76(O.69~0.84)和0.52(0.38—0.71)。结论亚洲成人SES与MS患病风险有关联,受教育水平高的女性MS患病风险较低,收入水平高的男性MS患病风险较高。Objective The aim of this study is to quantify the association between socioeconomic status (SES) and metabolic syndrome (MS) in Asian adults using Meta-analysis. Methods A systematic search was performed for cpidemiological studies on the association of SES (educational level, income, occupation) and MS, published to Sep. 16, 2014 on Pubmed, Embase, Sinomed, CNKI and Wanfang databases. Literature search, study selection and data extraction were conducted by two investigators independently. All statistical analyses were performed using Stata version 12.0. Results A total of 22 studies including 4231 158 adults fulfilled the inclusion criteria. Compared with persons with a lower educational level, those with a higher educational level were less likely to have MS (Middle: OR=0.82, 95%CI: 0.76~0.90 and High: OR=0.66, 95%CI: 0.56-0.78). When stratified by sex, the reverse relation between educational level and MS was notable in females (Middle: OR=0.76, 95%CI: 0.69-0.84 and High: OR=0.52, 95%CI: 0.38-0.71), but not in males. In addition, income level was in parallel with the prevalence of MS in males (Middle: OR=l.30, 95%CI: 0.98-1.74 and High: OR=1.44, 95%CI: 1.15-1.81), but not in females. Conclusions There was a significant relationship between SES and MS in Asian adults. Females with high educational level were associated with decreased riskof MS, while males with high income level were associated with increased risk of MS.
