调脂通络饮联合阿托伐他汀片对合并高脂血症的2型糖尿病患者血脂代谢的影响

摘　　要：目的：探讨调脂通络饮联合阿托伐他汀片对合并高脂血症的2型糖尿病患者血脂代谢的影响。方法本研究为随机对照研究。将符合纳入标准的患者按随机数字表法分为2组，患者均控制饮食、合理运动，常规口服磺脲类加双胍类药物使血糖控制在正常范围。对照组在此基础上加服阿托伐他汀片，20 mg/d；观察组在对照组治疗基础上加服调脂通络饮，2组均治疗6个月。比较2组治疗前后血糖及血脂水平的变化，并以此为基础评价临床疗效。结果观察组和对照组总有效率分别为87.5%（56/64）和73.0%（46/63），2组比较差异有统计学意义（χ2＝4.213，P＝0.040）。治疗后，观察组血清总胆固醇[（4.31±1.11）mmol/L比（5.62±1.34）mmol/L，t＝6.004]、三酰甘油[（1.73±0.53）mmol/L比（2.57±1.01）mmol/L， t＝5.855]均较对照组明显降低（P＜0.01）；空腹血糖[（6.31±1.64）mmol/L 比（7.21±1.71）mmol/L，t＝3.027]、餐后2 h血糖[（8.81±1.39）mmol/L比（9.69±1.55）mmol/L，t＝3.370]和糖化血红蛋白[（6.79±1.61）%比（7.96±1.72）%，t＝3.958]较对照组治疗后明显下降（P＜0.01）。结论调脂通络饮联合阿托伐他汀片可有效降低合并高脂血症的2型糖尿病患者血清总胆固醇、三酰甘油，其降血脂效果优于单纯服用阿托伐他汀片。Objective To observe the effects ofTiaozhi-Tongluo decoction combined with atorvastatin tablets on type 2 diabetes mellitus and lipid metabolism in patients with hyperlipidemia.Methods All patients were recruited into a control group and an observation group. Patients in both groups were performed diet control, reasonable exercise, and routine oral sulfonylureas and biguanides to make glucose level in a normal range. The control group was treated with atorvastatin tablets, 20 mg/d. While the observation group was treated with atorvastatin tablets plusTiaozhi-Tongluo decoction. After both groups were treated for 6 months, the blood glucose and blood lipid level were detected for the evaluation of clinical curative effect.Results The total effective rates in the observation group and the control group were 87.5%（56/64）and 73.0%（46/63）, respectively, the difference was statistically significant （χ2=4.213,P=0.040）. The serum levels of total cholesterol （4.31 ± 1.11 mmol/Lvs.5.62 ± 1.34 mmol/L,t=6.004） and triglyceride （1.73 ± 0.53 mmol/Lvs. 2.57 ± 1.01 mmol/L,t=5.855） in the observation group were significant lower than those in the control group. The levels of fasting plasma glucose （6.31 ± 1.64 mmol/Lvs.7.21 ± 1.71 mmol/L,t=3.027） , 2-h postchallenge plasma glucose （8.81 ± 1.39 mmol/Lvs.9.69 ± 1.55 mmol/L,t=3.370）, and glycosylated hemoglobin （6.79% ± 1.61%vs.7.96% ± 1.72%,t=3.958） in the observation group after treatment were significantly decreased than the control group （P〈0.01）.ConclusionTiaozhi-Tongluodecoction combined with atorvastatin can reduce serum total cholesterol and triacylglycerol, and it is superior to atorvastatin alone in reducing lipidemia in type 2 diabetes mellitus patients with hyperlipidemia.
