摘 要:目的:探讨新诊断2型糖尿病患者糖负荷后胰升糖素样肽-1(GLP-1)水平的变化。方法对30例新诊断2型糖尿病患者(糖尿病组)和10例健康志愿者(对照组)行口服葡萄糖耐量试验(OGTT)。分别于空腹,口服葡萄糖后30、60、120和180 min 各采集静脉血10 mL,测定血糖(葡萄糖氧化酶法)、血浆胰岛素(电化学发光免疫法)和GLP-1(7~36,酶联免疫吸附法)。观察 OGTT 中各时间点血糖、血浆胰岛素和 GLP-1(7~36)水平的变化。结果糖尿病组患者在 OGTT 过程中各时间点(空腹,餐后30、60、120和180 min)的血糖水平均高于对照组(均 P <0.05)。糖尿病组空腹及口服葡萄糖后120 min 胰岛素水平与对照组比较差异无统计学意义(P >0.05);糖尿病组口服葡萄糖后胰岛素水平于60 min 达峰值,对照组胰岛素水平于30 min 达峰值,糖尿病组胰岛素峰值水平均低于对照组(P <0.05);糖尿病组口服葡萄糖后180 min 胰岛素水平高于对照组、胰岛素曲线下面积则低于对照组(均P <0.05)。糖尿病组空腹和口服葡萄糖后各时间点的 GLP-1水平及曲线下面积与对照组比较差异无统计学意义(P >0.05)。结论新诊断2型糖尿病患者口服葡萄糖后 GLP-1(7~36)水平无异常。ABSTRACT:Objective To investigate the changes in glucagon-like peptide-1 (GLP-1)secretion after glucose load in patients with newly diagnosed type 2 diabetes.Methods Oral glucose toler-ance test (OGTT)was performed in 30 patients with newly diagnosed type 2 diabetes (diabetes group)and 10 healthy volunteers (control group).Venous blood samples (10 mL)were collected in the fasting state and at 30,60,120 and 180 minutes after oral glucose uptake.Plasma levels of glucose,insulin and GLP-1 (7-36)were measured using glucose oxidase method,electrochemilu-minescence immunoassay and enzyme linked immunosorbent assay,respectively.Results Com-pared with control group,plasma glucose levels significantly increased in diabetes group at all time points (P <0.05).There were no significant differences in insulin levels between the two groups in the fasting state and at 120 minutes after oral glucose uptake (P >0.05).Plasma insulin levels reached the peak at 60 minutes after oral glucose uptake in diabetes group and at 30 minutes after oral glucose uptake in control group.The peak insulin levels in diabetes group were signifi-cantly lower than those in control group (P <0.05).In addition,compared with control group,in-sulin levels increased at 180 minutes after oral glucose uptake and area under the curve (AUC)of insulin decreased in diabetes group (P <0.05).No significant differences in levels and AUC of GLP-1 between the two groups at all time points (P >0.05).Conclusion No changes in GLP-1 (7-36)levels occur after oral glucose load in patients with newly diagnosed type 2 diabetes.
