超声引导G18套管针穿刺在不充分充盈膀胱造瘘中的应用

摘　　要：目的探讨B超引导下G18套管针穿刺在不充分充盈膀胱造瘘中的有效性及安全性。方法对2例后尿道断裂患者，4例球部尿道断裂，1例腰椎手术术前引流尿液患者和2例神经外科手术术前引流尿液患者，因膀胱均不充分充盈，B超检查尿量约100mL，耻骨上区扪不到充盈膀胱，采用B超定位下G18套管针穿刺膀胱，穿刺成功后注入生理盐水200～400mL，充分充盈膀胱，然后再行膀胱穿刺造瘘。结果9例患者均穿刺造瘘成功，手术时间10～15min，未出现肠管损伤、前列腺损伤等并发症。结论采用B超定位下G18套管针穿刺行不充分充盈膀胱造瘘术，具有操作简单、安全有效等优点。Objectives To study the effective and safety of the use of GI8 trocar needle puncture guided by B ultrasound in cystostomy of inadequate filled bladder. Methods The patients with inadequate filled bladder, including 2 cases of posterior urethral rupture, 4 cases of bulbar urethral rupture, 1 case of preoperative drainage of u- rine before lumbar operation ; 2 casesof preoperative drainage of urine before craniocerebral operation, received su- prapubie eystostomy using the 18G troear needle puncture guided by B ultrasound from November 2012 to June 2014. Checked by B ultrasound,the capacity of bladder was about 100ml and the bladder couldnt be percussed in suprapu- bic area in all patients. 200 -400ml saline was injected to the inadequate filled bladder after successful puncture and then cystostomy was done after the bladder fully filled. Results All patients punctured successfully. Operation time differed from 10 to 15 minutes. No complication such as bowel injury and prostate injury happened. Conclu- sions It is simple and safe to use the G18 trocar needle puncture guided by B ultrasound for the suprapubic cys- tostomy in inadequate filled bladder.
