银杏叶提取物联合直肌巩膜深层埋藏术治疗视网膜色素变性的临床研究

摘　　要：目的：观察银杏叶提取物联合直肌巩膜深层埋藏术治疗视网膜色素变性的疗效及安全性。方法：设计回顾性比较性病例系列。将研究对象60例（120眼）视网膜色素变性患者分为观察组和对照组。30例（60眼）应用银杏叶提取物联合直肌巩膜深层埋藏术治疗视网膜色素变性。术前1周及术后1周应用银杏叶提取物20m L·70mg-1·d-1。对照组30例（60眼）单纯应用银杏叶提取物浓度20m L·70mg-1·d-1治疗治疗视网膜色素变性。主要指标有视力、视野、视网膜电图（ERG）等检查结果。术后1周、1月、3个月、6个月复查,随访12个月~3年。结果：观察组有效率75%,对照组有效率45%,两组比较有显著性差异。治疗后两组比较视力提高,视野扩大,视网膜电图可见波形及a、b波幅值增加,观察组各指标均较对照组比较差异有显著性（P〈0.05）。结论：银杏叶提取物联合直肌巩膜深层埋藏术综合治疗视网膜色素变性是安全、有效的,能在一定程度上改善视网膜色素变性（RP）尤其是合并血管狭窄或闭塞患者的视力、视野等视功能,从而改善患者的生存质量。Objective： To observe the efficacy and safety of buried in treatment of retinal pigment degeneration of the Ginkgo biloba extract combined rectus scleral deep. The study of 60 cases（120 eyes） of patients with retinitis pigmentosa. Methods： the patients were divided into the treatment group and control group. In 30 cases （60 eyes）application of Ginkgo biloba extract combined rectus scleral deep buried in treatment of retinitis pigmentosa. Before operation, 1 week and 1 week after operation the application of Ginkgo biloba extract concentration （20mL / day, 70mg）. A control group of 30 cases （60 eyes） the simple application of Ginkgo biloba extract concentration （20mL / day, 70mg） therapy for the treatment of retinitis pigmentosa. Main indicators of visual acuity, visual field, （ERG） examination results. In January, 1 weeks and 3 months after surgery,6 months, followed up for 12 monthsto 3 years. Results： the effective rate of the treatment group 75%, the control group was 45%. The 2 groups had significant difference. Visual acuity improved after treatment were compared between the two groups, to broaden our horizons, ERG and visible waveform a, b wave amplitude increased, the treatment the group compared with the control group, the indexes had significant difference （P〈0.05）. Conclusion： Ginkgo biloba extract combined rectus scleral deep buried in treatment of retinitis pigmentosa with comprehensive treatment is safe and effective, and can improve the retinal pigment degeneration to a certain extent （RP） especially with vascular stenosis or occlusion in patients with visual acuity and visual field visual function, so as to improve the quality of life of patients.
