结核性胸膜炎在内科胸腔镜下分型的研究

摘　　要：目的研究结核性胸膜炎在可弯曲电子胸腔镜下表现与病理表现,进一步评价可弯曲电子胸腔镜在结核性胸膜炎中的分型诊断价值,提高结核性胸膜炎的诊断水平。方法对2007年8月-2013年8月天津市海河医院结核科98例通过可弯曲电子胸腔镜检查结合临床及病理确诊结核性胸膜炎患者进行回顾性研究,综合分析其临床表现、影像学特点、胸腔镜下特征、病理学表现,对其进行分型。结果可分为胸膜充血型、结节型(弥漫小结节型、大小结节混合型、大结节型)、干酪坏死型(包括脓胸1例)、纤维素粘连型与混合型。结论可弯曲电子胸腔镜可协助结核性胸膜炎的诊断分型,指导治疗,评估预后。[ Objective ] The purpose of this study was to investigate the tuberculous pleurisy patient＇s charac- teristics via flexible electronic thoracoscopy and the pathological properties and to evaluate the value of flexible elec- tronic thoracoscopy in the classification of tuberculous pleurisy and improving diagnostic level. [ Methods ] The clin- ical,radiological,pathological data and the characteristics via flexible electronic thoracoscopy of 98 cases of tubercu- lous pleurisy who was performed flexible electronic thoracoscopy were retrospectively analyzed and classified in Tianjin Haihe Hospital from August 2007 to August 2013. [Results ] It was showed these types including hyperemia and edema of pleura type, nodules type （diffused miliary nodules, small and big mixed nodules, big nodules）, caseous necrosis （pyothorax） type, pleural fibrinous septa or adhesion type and mixed type. [ Conclusion ] The flexible elec- tronic thoracoscopy can help diagnose the type of tuberculous pleurisy ,conduct the treatment and evaluate the prognosis.
