摘  要:目的:研究细胞增殖与凋亡在肌腱退行性变病理机制中的作用。方法:本研究利用SD大鼠8周下坡跑过度使用模型造成肱二头肌长头肌腱结节沟段退行性变,采用病理切片、免疫组化、免疫荧光等方法检测了不同运动阶段肌腱损伤、细胞增殖与凋亡情况。结果:大鼠肱二头肌肌腱结节间沟段病理总评分呈逐渐升高趋势,在第4、6周达到最高(P<0.01),其中以胶原纤维排列、细胞核形态、细胞分布、胶原可染性的变化最为显著(P<0.05);肌腱细胞凋亡在运动第2周出现显著升高(P<0.05),第4周达到并一直维持较高水平(P<0.01);各时间点均未发现明显肌腱细胞增殖现象。结论:大鼠下坡跑运动可以导致肱二头肌肌腱结节间沟段发生退行性改变;肌腱细胞过度凋亡和增殖不足是过度使用导致肌腱退行性变过程中的重要病理变化。Objectives To study the role of cell proliferation and apoptosis in overuse-induced tendon.Methods Overuse tendon model of SD rat was established through 8-week downhill running on a treadmill.The pathological changes and growth factor protein expression in various stages of overuse were detected by immunohistochemistry and immunofluorescence method. Results(1)Pathological score gradually increased with prolonging of overuse duration and reached the highest at 4-6 weeks(P 〈 0.01),in which the most significant changes in arrangement of collagen fibers,nuclear morphology,tenocytes distribution,and collagen dyeability revealed(P 〈 0.05).(2)Tenocytes apoptosis increased significantly after 2 weeks(P 〈 0.05),and reached the highest at 4-8 weeks of running(P 〈 0.01). There was no proliferation of tenocytes during the whole process.Conclusion(1)Downhill running causes degenerative changes in tendon of inntertubercular sulcus of biceps brachii muscle in rats.(2)Excessive apoptosis and insufficient proliferation of tenocytes serve as an important pathological phenomenon of tendon degeneration in overuse and lead to the decrease in the number of tenocytes,and thus weaken the ability of tendon repair.
