北京市朝阳区2010—2012年社区基本公共卫生服务提供变化情况分析

摘　　要：目的了解北京市朝阳区2010—2012年社区卫生服务机构基本公共卫生服务提供的现状、变化趋势及存在的问题,为社区卫生服务的良好发展提供依据。方法 2013年12月课题组通过朝阳区的社区卫生服务机构网络直报和管理信息平台,结合社区卫生服务机构常规月报、季报、年报和临时报表等形式的有关统计信息,收集2010—2012年朝阳区社区卫生服务机构公共卫生服务提供的相关信息,并进行分析。共收集了42个社区卫生服务中心,199个社区卫生服务站的相关数据。结果与2010年相比,2012年朝阳区社区卫生服务机构全科(4 882 487人次比3 342 681人次)、中医(1 597 939人次比1 141 989人次)等特色门诊服务量增加,年均增长率分别为20.9%、18.3%;慢性病管理总人数(487 484人比294 274人)增加,年均增长率为28.7%;开展健康教育讲座(2 467次比2 325次)、健康促进活动(1 800次比1 043次)增加,年均增长率分别为3.0%、31.4%;参加健康教育讲座(142633人次比86 183人次)、健康促进活动(231 425人次比121 024人次)的社区居民亦增加,年均增长率分别为28.6%、38.3%;其他基本公共卫生服务的提供如结核病免费服药、精神疾病免费服药、儿童保健、孕产妇保健等亦增加。2012年家庭健康档案电子化率达92.4%(1 091 266/1 180 767),个人健康档案电子化率达91.5%(2 568 591/2 796 941),但仍存在总体服务的覆盖率较低,难以满足社区居民需求;发展速度不足以实现规划目标;部分服务的提供量出现下滑;健康教育与健康促进服务模式单一等问题。结论 2010—2012年间,朝阳区社区卫生服务机构各项基本公共卫生服务都有了较大的发展,但亦存在有不合理的地方。针对目前的现状,朝阳区应制定系统的社区公共卫生发展规划;加强人才引进的力度;增加财政的支持力度;改善社区机构的条件;加强各双向转诊制度的执行;发挥社区公共卫生居�Objective To investigate the current status, changing trend and existing problems of the basic public health services provided by community health service settings in Chaoyang District of Beijing between 2010 and 2012 so as to provide some suggestions for the promotion of community public health services. Methods In December 2013,the relevant information of the community public health services in Chaoyang District of Beijing between 2010 and 2012 was collected from network direct reporting, information platform and monthly, quarterly, annual and interim reports. Data from 42 community health service centers and 199 community health service stations were collected and analyzed. Results Compared with that in 2010,the outpatient service volume of general practice（4 882 487 vs. 3 342 681） and traditional Chinese medicine（1 597 939 vs. 1 141 989）increased significantly with annual increase rate at 20. 9% and 18. 3%;the number of people under management of chronic diseases（ 487 484 vs. 294 274 ）increased by 28. 7%;lectures of health education（ 2 467 vs. 2 325 ） and health promotion activities（1 800 vs. 1 043）flourished with annual increase rate at 3. 0% and 31. 4%;the number of residents who participated in the lectures of health education（ 142 633 vs. 86 183 ）and health promotion activities（ 231 425 vs. 121 024 ） increased by 28. 6% and 38. 3% annually;other basic public health services also developed in a gradual manner,such as the free medicine for TB patients and psychotic,child health care and maternal care;the rate of electronic health records reached 92. 4% for family（1 091 266 vs. 1 180 767）and 91. 5% for individual（2 568 591 vs. 2 796 941）in 2012. However,there were also problems：the coverage rate of the services was low and it was difficult to meet the needs of community residents,the development was not rapid enough to achieve the planning objectives,part of the services showed decline in provision,and the model of health education and health promotion services was not d
