感染可能性评分预测血液系统肿瘤患者血流感染的可行性研究

摘　　要：目的通过感染可能性评分(IPS)对血液系统肿瘤患者发生血流感染(BSI)进行预测,对其可行性进行研究,为血液系统肿瘤患者发生血流感染的预防提供参考依据。方法对84例血液恶性肿瘤患者在进行化学治疗前均进行一次感染可能性评分,对所有感染可能性评分的患者从入院到出院或者是出现死亡均进行血流感染率的监测,评价感染可能性评分诊断血流感染的临床价值。结果感染可能性评分所有患者评价为(12.02±6.14)分,血流感染患者9例,发生率为11.54%;发生BSI患者的感染可能性评分为(14.08±6.96)分,未发生BSI患者的感染可能性评分为(10.98±6.12)分,两者比较差异有统计学意义(P<0.05);感染可能性评分预测血流感染的最佳值为12.98、灵敏度为78.69%、特异度为75.77%、曲线下面积为0.69、阳性预测值为66.14%、阴性预测值为92.35%。结论感染可能性评分作为筛选血液系统肿瘤患者血流感染的一个有用工具,对患者的护理、治疗有重要的作用。OBJECTIVE To study the feasibility of infection possibility score in prediction of bloodstream infections in the hematological cancer patients so as to provide guidance for the prevention of bloodstream infections in the hematological cancer patients .METHODS The infection possibility scoring was carried out for 84 hematological cancer patients before the chemotherapy ,then the incidence of bloodstream infections was monitored for all the pa‐tients taking the infection possibility scoring from admission to discharge or till death ,and the clinical value of the infection possibility scoring in diagnosis of the bloodstream infections was evaluated .RESULTS The infection possi‐bility score of all the patients was （12 .02 ± 6 .14） points ,the bloodstream infections occurred in 9 cases ,with the incidence rate of 11 .54% ;the infection possibility score of the patients with bloodstream infections was （14 .08±6 .96） points ,the patients without bloodstream infections （10 .98 ± 6 .12） points ,and there was significant differ‐ence （P〈0 .05） .The optimal value of the infection possibility score in prediction of bloodstream infections was 12 .98 ,with the sensitivity of 78 .69% ,the specificity 75 .77% ,the area under the curve 0 .69% ,the positive pre‐dictive value 66 .14% ,the negative predictive value 92 .35% .CONCLUSION The infection possibility scoring can be used as a tool for the screening of bloodstream infections in the hematological cancer patients ,and it plays an important role in the nursing and treatment of the patients .
