T型连贯式无菌台布局在骨科下肢手术中的应用

摘　　要：目的探讨T型连贯式无菌台布局在骨科下肢手术中的应用,以减少随意放置无菌台带来的无菌物品污染现象,加强骨科手术切口感染手术室环节控制;规范手术组人员站位提高术中配合质量。方法选取2013年7-9月骨科下肢手术120例为常规组,术中无菌台的布局和人员站位未作规定,根据传统习惯无菌台放置在手术床靠尾端周围,洗手护士和器械师站在无菌台前,选取2013年10-12月骨科下肢手术122例为改进组,将手术床与无菌台放置成T型,规定洗手护士和器械师的站位在无菌台前,与手术医师面向同一方向,比较两组术中无菌物品的污染次数和手术医师对手术配合满意度。结果与传统习惯无菌台放置相比,T型连贯式无菌台布局的术中无菌物品的污染次数明显减少,常规组污染率为27.5%,改进组为8.20%,两组比较差异有统计学意义(P<0.01)。结论通过对下肢骨科手术采取T型连贯式无菌台布局和规范手术人员站位,使器械传递过程中避免了触及和跨越非无菌区域,从而减少了术中无菌物品的污染,同时使洗手护士更直观观察到手术进程,提高了手术配合质量。OBJECTIVE To study the application of T‐type coherent aseptic table′s layout in the lower limbs opera‐tions of orthopedics so asto reduce the incidence of pollution of aseptic items due to casual placement of the aseptic tables ,enhance the link control of operating room to prevent the surgical incision infections ,and standardize the stances of the operation staff to improve the quality of intraoperative cooperation .METHODS A total of 120 pa‐tients who underwent the lower limbs operations of orthopedics from Jul 2013 to Sep 2013 were assigned as the control group ,the aseptic table′s layout and the staffs′stances were not stipulated ,the aseptic table was placed a‐round the end of the operation bed ,and hands‐cleaned nurses and equipment staffs stood in front of the aseptic ta‐ble;while 122 patients who underwent the lower limbs operations of orthopedics from Oct 2013 to Dec 2013 were set as the improvement group ,the operation bed and aseptic table were placed to form a T type ,the hands‐cleaned nurses and equipment staffs were required to stand in front of the aseptic table ,facing the same direction as the surgeons .The number of times of contamination of the aseptic items during the surgery and the surgeons′satisfac‐tion with the surgical cooperation were observed and compared between the two groups .RESULTS As compared with the conventional placement of the aseptic table ,the T‐type coherent aseptic table′s layout has remarkably less times of contamination of intraoperative aseptic items ,the rate of contamination was 27 .5% in the control group , 8 .20% in the improvement group ,and there was significant difference between the two groups （P〈0 .01） . CONCLUSION The application of the T‐type coherent aseptic table′s layout in the lower limbs operations of ortho‐pedics and the standardized stances of the operation staffs can avoid touching or stepping over the non‐aseptic area so as to reduce the intraoperative contamination of the aseptic items ,making the hands‐c
