摘 要:目的探讨感染中枢神经系统肠道病毒(EV)的分子生物学分型。方法收集脑炎患儿脑脊液(CSF)样本72例,以EV71、COXB3、ECHO30标准病毒株为阳性对照,脑外科手术患儿CSF为阴性对照。采用通用引物和2种分型引物对CSF样本进行扩增,并对扩增产物进行分型研究。结果利用通用引物1次和2次PCR EV阳性检出率分别为69.44%(50/72)、73.61%(53/72),差异无统计学意义(P>0.05);对VP1段进行PCR扩增,1次和2次PCR后,阳性检出率分别为26.42%(14/53)、62.26%(33/53),差异有统计学意义(P<0.01);对VP2段进行PCR扩增,1次和2次PCR后,阳性检出率分别为16.98%(9/53)、54.72%(29/53),差异有统计学意义(P<0.01);2种分型引物对EV RNA阳性检出率差异无统计学意义(P>0.05);EV阳性样本与EV标准毒株相关序列同源性均在97%以上。结论利用分型引物能够检测临床常见EV的大多数血清型,可为防治EV引起的中枢神经系统感染提供临床参考。Objective To explore the molecular biological types of enterovirus(EV) that could infect central nervous system(CNS). Methods A total of 72 samples of cerebrospinal fluid(CSF) from children with encephalitis were collected,with standard viral strain EV71,COXB3 and ECHO30 as positive control and CAF from children underment brain surgery as negative control. The CSF samples were expanded by using common primers and 2 type primers so as to conduct type research on the expanded products. Results The positive detection rates once and twice after the application of reverse transcriptase polymerase chain reaction(PCR) with common primers were6 9. 4 4 %(5 0 / 7 2) and7 3. 6 1 %(5 3 / 7 2),respectively,but the difference was not significant(P〈 0. 05). PCR expansion was conducted to VP1 segment,and the positive detection rates once and twice after the application of PCR with common primers were 26. 42%(14 /53) and 62. 26%(33 /53),respectively(P〈 0. 01),which were 16. 98%(9 /53) and 54. 72%(29 /53) after the PCR expansion to VP2 segment(P〉0. 01). Moreover,there was no significant difference between the two primers in detecting EV(P〈 0. 05). The homology of relevant sequences in VE positive samples and EV standard viral strains was more than 97%. Conclusion The application of type primers can detect the most serum types of EV,thus providing clinical reference for the prevention of EV-induced CNS infection.
