吸烟对PPH术后吻合口愈合情况的影响

摘　　要：目的探讨吸烟对痔上黏膜环切吻合术(PPH)术后吻合口愈合情况的影响。方法选取2012年10月至2013年12月北京中医药大学东直门医院东区肛肠外科行吻合器PPH术后吸烟患者50例(吸烟组)和非吸烟患者50例(非吸烟组);术后3 d开始评估吻合口愈合情况,包括肛门疼痛、坠胀感、吻合口肉芽组织生长情况。结果术后非吸烟组患者治愈总有效率为92.0%;吸烟组治愈总有效率为88.0%,两组疗效比较差异无统计学意义(P>0.05)。非吸烟组在术后疼痛、坠胀感、吻合口肉芽组织生长分数显著低于吸烟组[0(0,1)比1(0,2)、0(0,1)比1(0,2)、0.5(0,2)比1(0,2),P<0.05]。结论吸烟能加重PPH术后肛门疼痛、坠胀感,并减缓吻合口肉芽组织生长。Objective To explore the impact of smoking on the union of anastomotic stoma after proce- dure for prolapse and hemorrhoidse （PPH） method. Methods The 50 smoking patients （ smoking group） and 50 non-smoking patients（non-smoking group） after PPH method were selected from Oct. 2012 to Dec. 2013 in the anorectal surgery ward of Eastern Distribution of Dong Zhimen Hospital affiliated to Beijing uni- versity of Traditional Chinese Medicine. The union of anastomatic stoma was assessed 3 days after the opera- tion, the content which included pain, feelings of sagging distention and granulation hyperplasia. Results The total healing rate of non-smoking group was 92. 0%. The total healing rate of smoking group was 88.0%. The total healing rate showed no statistically significant difference between non-smoking group and smoking group （P 〉 0.05 ）. But, on the aspects of pain feelings of sagging distention and granulation hyper- plasia, the union of anastomatic stoma showed that non-smoking group was obviously better than the smoking group[0（0,1） 比 1（0,2）,0（0,1） P6 1（0,2）,0. 5（0,2） It 1（0,2）,P 〈0.05]. Conclusion Smoking can add feelings of pain and sagging distention after PPH, and decelerate granulation hyperplasia.
