线状皮肤型红斑狼疮1例并文献复习

摘　　要：患者女,20岁。右下肢带状红斑、丘疹3个月。皮肤科情况：右足内侧往上至右腹股沟可见沿Blaschko线密集分布的暗红色至紫红色斑疹、丘疹,表面覆少许鳞屑。实验室检查：抗核抗体（ANA）1∶80（斑点型）,抗SSA抗体（3＋）,RF 43.8IU/m L（0~30IU/m L）。皮损组织病理示：表皮角化过度,可见毛囊角栓,灶状基底细胞液化变性,真皮浅、中层血管及附属器周围较多淋巴细胞浸润,皮下脂肪可见少量淋巴细胞浸润。诊断：线状皮肤型红斑狼疮。本文就其病因、临床表现、治疗进展及预后进行了文献复习。A 20-year-old female presented with zonal erythema and papules on her right lower limb for three months. Clinical characteristics revealed zonal, dense, dark red to purple macules and papules covered with a few scales along with Blaschko＇s lines from the medial aspect of the foot to her right groin. Laboratory examina- tions ： antinuclear antibodies （ANA） were 1 ： 80 （ speckled pattern）, anti-SSA antibodies （ 3 ＋）, RF 43.8IU/mL（0 ~ 30IU/mL）. Histopathology showed hyperkeratosis, follicular plugging and vacuolar alteration of the epidermal basal cell layer. Lymphocytes infiltrate in the perivascular and periadnexal in the superficial and medial dermis, and infiltration of few lymphocytes in the adipose tissue were observed also. The patient was diagnosed with linear cutaneous lupus erythematosus. Its etiology, clinical manifestation, treatments and prog- nosis were reviewed.
