摘  要:目的探讨乌司他丁对劳力性热射病(exertional heatstroke,EHS)血清中白介素-17(IL-17)、高敏C-反应蛋白(HS-CRP)、心肌肌钙蛋白(c Tn I)、β2-微球蛋白(β2-MG)含量水平的影响。方法采用ELISA、免疫荧光干式定量、微粒子酶免、比浊法测定31例EHS者加用乌司他丁治疗(观察组)与30例未加用者(对照组)入院时及入院第6天血清中IL-17、HS-CRP、c Tn I、β2-MG含量,并与健康人(正常组)作对照。同时对比分析观察组与对照组多脏器功能障碍综合征(multiple organ dysfunction syndrome,MODS)器官受累率、受累器官恢复时间。结果观察组及对照组入院时血清中IL-17、HS-CRP、c Tn I、β2-MG含量比较,差异无统计学意义(P>0.05),但均较正常组明显升高(P<0.01);观察组及对照组入院第6天血清中IL-17、HS-CRP、c Tn I、β2-MG含量均较入院时明显降低(P<0.01),但观察较对照组降低更显著(P<0.05)。观察组MODS器官受累率低于对照组(P>0.05),受累器官恢复时间明显提前(P<0.05)。结论乌司他丁通过抑制细胞因子和炎性介质的产生与释放,减轻其对心、肾等功能的损害,避免了EHS者器官或系统受累,并促进受累器官或系统的快速恢复。Objective To investigate the impacts of ulinastatin on serum interleukin -1 7(IL -6),high sense C -reactive protein(HS -CRP),cardiac troponin(cTnI),beta 2 -microglobulin(beta 2 -MG)of patients with exertional heat stroke. Methods ELISA,immunofluorescence dry ration,microparticle enzyme immunoassay,turbidimetric method were used for de-termination of serum IL -1 7,HS -CRP,cTnI,beta 2 -MG levels of 31 cases of EHS additionally treated with ulinastatin(ob-servation group)and 30 cases without ulinastatin treatment(control group)on the second and sixth day of admission,and com-pared with healthy people(normal group).At the same time,the status of multiple organ dysfunction syndrome,involved organs and recovery time in observation and control groups were compared and analyzed.Results No significant difference was observed between control group and observation group in serum IL -1 7,HS -CRP,cTnI,beta 2 -MG level(P 〉0.05)at the time of ad-mission,but they were significantly higher in observation and control group than normal group(P 〈0.01 ).On the sixth day of admission,serum IL -1 7,HS -CRP,cTnI,beta 2 -MG in observation group were obviously lower than control group (P 〈0. 01 ).There was a significant difference in MODS organ involvement rate between the two groups(P 〉0.05)and the organ recov-ery time in the observation group was significantly shorter(P 〈0.05).Conclusion By inhibiting the production and release of cytokines and inflammatory mediators,ulinastatin could reduce the damage to the heart and kidney function,reduce EHS organs or systems involved rate,and promote the rapid restoration of affected organs or systems.
