双多普勒同步与非同步成像对心房颤动患者左心室舒张功能参数测量的对比分析  被引量:7

Analysis of dual-Doppler simultaneous technique and asynchronous technique in assessing left ventricular diastolic function in patients with atrial fibrillation

摘  要:目的探讨双多普勒同步成像技术(简称双多普勒模式)评估射血分数正常的心房颤动患者左室舒张功能的价值。方法随机选取40例射血分数正常的心房颤动患者为病例组,40例健康体检者为对照组。①采用双多普勒模式同步获取同一心动周期的二尖瓣口血流舒张早期峰值速度(E)和组织多普勒(TDI)二尖瓣环侧壁(L)心肌运动峰值速度(e’);②同步获取同一心动周期二尖瓣口E峰和TDI二尖瓣环室间隔(s)心肌运动峰值速度(e’);③同步获取同一心动周期二尖瓣口E峰和舒张期二尖瓣口血流播散速度(Vp)。计算E/e’(L)、E/e’(S)及E/Vp。结果与对照组比较,病例组E/e’(L)、E/e’(s)及E/Vp均增高,差异有统计学意义(P〈0.05)。Bland—Altman分析显示双多普勒模式较传统模式测量E/e’(L)、E/e’(s)、E/Vp的重复性更好,组内相关系数(ICC)明显高于传统模式。结论双多普勒模式用于评估心房颤动患者左室舒张功能较传统模式的重复性更好,结果更准确。Objective To evaluate the value of dual-Doppler imaging technology (referred to as dual- Doppler modality for short) in assessing left ventricular diastolic function in patients with atrial fibrillation (AF) and normal left ventricular ejection fraction (LVEF). Methods A total of 40 patients with AF and normal LVEF were enrolled as the AF group, and 40 healthy volunteers were composed of the control group. (1)Peak early diastolic transmitral flow velocity (E) and tissue Doppler lateral (L) mitral annular early diastolic velocity (e') were measured simultaneously in the particular eardiac cycle by dual-Doppler modality. QPeak early diastolic transmitral flow velocity (E) and tissue Doppler septal (S) mitral annular early diastolic velocity (er) were measured simultaneously in the particular cardiac cycle. (3)Peak early diastolic transmitral flow velocity (E) and color M-mode Doppler flow propagation velocity (Vp) were measured simultaneously in the particular cardiac cycle. Then E/er (L),E/er (S) and E/Vp were calculated, respectively. Results Compared to the control group, E/er (L), E/ er (S)and E/Vp were all higher in AF group ( P〈0.05).Bland-Altman showed that E/er (L), E/ e' (S)and E/Vp measured by the dual-Doppler modality had better reproducibility and higher intraclass correlation coefficient (ICC) than the conventional Doppler modality. Conclusions The dual-Doppler modality is valuable for evaluating left ventricular diastolic function in patients with AF and has better reproducibility and more accurate results than the conventional Doppler modality.
