牵正压推法配合展筋接骨方口服治疗闭合性三踝骨折临床研究

摘　　要：目的:观察牵正压推法配合口服展筋接骨方治疗闭合性三踝骨折的临床疗效。方法:回顾性分析2009年1月-2012年5月收治闭合性三踝骨折患者58例的临床资料,其中治疗组(采用牵正压推法配合口服展筋接骨方,外敷消肿化瘀方)与对照组(采用手术切开复位内固定术)各29例,两组患者均采用美国足踝协会踝与后足功能(American Orthopaedic Foot&Ankle Society,AOFAS)评分标准与Leeds影像学评定标准对两组患者临床疗效进行比较。结果:8周后对踝关节骨折临床愈合情况按照Leeds评定标准,治疗组的优良率为89.66%,对照组的优良率为75.86%;平均16个月后随访,对踝关节功能按照AOFAS评定标准,治疗组的优良率为93.10%,对照组的优良率为82.76%,两组比较差异有统计学意义(P<0.05)。结论:采用牵正压推法配合口服展筋接骨方,外敷消肿化瘀方治疗闭合性三踝骨折疗效确切。Objective：To detect the clinical efficiency of holding positive pressure pushing method combined with Zhanjin Jiegu decoction to treat closed fracture of three ankles.Methods：Retrospective analysis of 58 patients with closed fracture of three ankles from January 2009 to May 2012.They were divided into treatment group which were treated by holding positive pressure pushing method combined with Zhanjin Jiegu decoction and Xiaozhong Huayu decoction and control group which were treated by open reduction and internal fixation,with 29 cases in each group.The two groups were compared by evaluation scores of the standard of American Orthopaedic Foot ＆ Ankle（AOFAS）and Leeds image.Results：The excellent and good rate of clinical healing in treatment and control groups were 89.66% and 75.86%respectively according to Leeds standard 8 weeks after treatment.The excellent and good rate of ankle joint function in treatment and control groups were 93.10% and 82.76% respectively according to AOFAS standard after 16 months follow-up（P〈0.05）.Conclusion：The holding positive pressure pushing method combined with Zhanjin Jiegu decoction oral and Xiaozhong Huayu decoction external application are effective for treating closed fracture of three ankles.
