双低法320排CT左心室功能分析可行性研究

摘　　要：目的探讨低辐射剂量和低对比剂剂量在320排容积CT左心室功能评价中的可行性。方法对临床拟诊为冠心病,应用320排容积CT冠脉CTA检查后排除冠心病的患者,按照不同扫描条件随机分为3组(60例/组,共180例)。A组:120kV,碘海醇350mgI/ml;B组:120kV,碘克沙醇270mgI/ml;C组:100kV,碘克沙醇270mgI/ml。将3组冠脉CTA图像质量、心功能数据、左室噪声及对比噪声比、有效辐射剂量及平均碘剂量等进行统计学分析比较。结果三组间冠脉CTA图像质量差异无统计学意义(P>0.05)。A组与B组各心功能数据、噪声及对比噪声比和辐射剂量间的差异均无统计学意义(P>0.05),碘平均剂量的差异有统计学意义(P<0.05);B组和C组各心功能数值、噪声、碘平均剂量间的差异无统计学意义(P>0.05),有效辐射剂量及对比噪声比的差异有统计学意义(P<0.05)。结论双低法一站式320排容积CT可以获得可靠的CTA图像和心功能参数,为临床提供更多的诊断信息。Objective To evaluate the feasibility of the clinical application in left ventricular （LV ） function analysis by 320‐row volume CT with low radiation dose and lower dosis of contrast medium （double low ） .Methods Patients with suspected coronary heart disease ,who were then ruled out by the diagnosis with 320‐row volume coronary CTA ,were di‐vided into three groups randomly （60 cases per group） .Group A ：120 kV ,Iohexol 350 mgI/ml;Group B：120 kV ,Iodix‐anol 270 mgI/ml;Group C ：100 kV ,Iodixanol 270 mgI/ml .Some parameters at three interblock ,such as the quality of the coronary CTA images ,numerus of LV function ,contrast to noise ratio （CNR） of LV ,effective dose （ED） ,and the average iodine quantity of contrast medium ,were statistically analyzed respectively .Results There were no significant differences in the CTA image quality among the three groups ,and no differences were found in the cardiac function ,noise , CNR ,and ED between the group A and group B（ P〉0 .05） .However ,significant differences were found in ED and CNR between the group B and group C（ P 〈0 .05） ,with on other differences being found （ P 〉0 .05） .Conclusion LV func‐tion can be reliably analyzed by “One‐stop” 320‐row volume CT with double low ,in which more diagnostic information could be provided to clinic .
