经眉皮肤入路外伤性前颅底缺损修复重建术

摘　　要：目的探索经眉皮肤入路修复重建外伤性前颅底缺损伴有脑组织疝入眼眶内的手术方法。方法回顾性分析2013年6月经眉皮肤入路修复重建外伤性前颅底缺损术1例。经眉皮肤入路,沿眶上壁骨膜下向眼眶深面剥离至颅底破损处,清除颅骨碎片及疝出的脑组织、病变软组织,显露颅底骨缺损。塑形重建钛网后,人工硬脑膜缝扎固定在钛网上,眶上缘处钛钉固定钛网,手术创口分层缝合。结果患者手术成功,术后2年随访眼球突度正常,眼功能良好,脑功能无异常。结论合理的选择手术适应证,经眉皮肤入路修复重建外伤性颅底缺损术是安全可行的。Objective The purpose of the study is to evaluate surgical principle of the eyebrow approach for repair and reconstruction of traumatic anterior skull base defect accompanied with brain herniation. Methods A patient with traumatic anterior skull base defect was retrospectively analyzed, By using an eyebrow approach, dissected orbital subperiosteally, the defect of the skull base was exposed and the skull fracture fragments was cleared. The titanium mesh was shaped to fix the skull base defect. The titanium mesh was screwed the on the supraorbital after the dural sutured. Finally, operation wound was sutured in layers. Results The operation was successful and the patient recovered. After two-year follow-up, the patient has normal eye protrusion and brain function. Conclusion The eyebrow aooroach is safe and feasible for reconstruction of skull base defect.
