摘　　要：目的探讨计划性剖宫产、自然分娩、自然分娩变异剖宫产对产妇产后抑郁的影响。方法将935例产妇按照分娩方式分为计划性剖宫产组472例、自然分娩组342例及自然分娩变异剖宫产组121例。比较3组产妇产后抑郁发生率及Beck抑郁自评问卷调查结果。结果自然分娩组(13.74%)、计划性剖宫产组(17.58%)产后抑郁率均低于自然分娩变异剖宫产组(44.63%),差异均有统计学意义(P<0.05);与产前1天比较,术后3天计划性剖宫产组、自然分娩变异剖宫产组BDI评分均升高,差异均有统计学意义(P<0.05);与术后3天比较,术后1周计划性剖宫产组BDI评分显著降低,差异有统计学意义(P<0.05),自然分娩变异剖宫产组评分无显著变化,差异无统计学意义(P>0.05)。结论计划性剖宫产产妇产后抑郁在术后1周得到缓解,自然分娩变异剖宫产产妇产后抑郁发生率较高,且以中、重度抑郁为主。Objective To investigate the influence of planned cesarean section, natural childbirth, cesarean section delivery variation on postpartum depression. Methods According to the different modes of delivery, 935 parturients were divided into planned cesarean section group （n= 472）, vaginal delivery group （n= 342） and the variation of natural deliv- ery cesarean section group （n= 121）. The incidence of postpartum depression and Beck Depression Inventory results （BDI） were compared among the 3 groups. Results The postpartum depression rates o f natural childbirth group （13.74% ） and planned cesarean section group （17.58 % ） were lower than that of the natural childbirth variation in cesar- ean section group （44.63%） （P〈0.05）. At prenatal ld and postoperative 3d, the BDI score of the natural delivery ce- sarean group variation was higher than that of planned cesarean section group （P〈0.05）. Compared with that of planned cesarean section group, BDI score postoperative 3d of planned cesarean section group decreased significantly （P〈0.05）. The natural childbirth variation in cesarean section group scores did not change significantly （P〉0.05）. Conclusion The postpartum depression of planned cesarean section group is alleviated within 1 week after operation. The postpartum depression rate of variation of natural delivery cesarean section group is much higher. Most of which is moderate and severe depression.
