摘  要:目的:通过观察针刀对内侧副韧带(MCL)生物力学特性及软骨整合素β1(Integrinβ1)、Ⅱ型胶原(Col-Ⅱ)基因表达的影响,探讨针刀"调筋治骨法"治疗膝骨关节炎的作用机制。方法:40只新西兰家兔随机分为空白组、模型组、针刀组、电针组。造模后,针刀组和电针组分别治疗3周。应用Bose Electro Force 3300疲劳试验机对各组MCL进行拉伸、蠕变及应力松弛等力学测试;应用Real-time PCR法检测各组软骨Integrinβ1、Col-Ⅱ基因表达水平。结果:1拉伸试验示模型组MCL的各项指标降低,与空白组相比,差异显著(P<0.01,P<0.05),干预后各指标均有上升趋势,针刀组除最大应力及弹性模量外,极限载入荷、最大位移和最大应变与模型组相比均有显著性差异(P<0.05);模型组蠕变率与空白组相比,差异显著(P<0.01),针刀组与模型组相比,差异显著(P<0.05);电针组MCL的各项力学指标与模型组相比均无显著性差异。2Real-time PCR结果示,造模后,模型组膝关节软骨Integrinβ1、Col-Ⅱm RNA表达量较空白组明显降低(P<0.01);针刀及电针干预后,Integrinβ1、Col-Ⅱm RNA表达量较模型组均明显升高(P<0.01,P<0.05)。结论:针刀"调筋治骨"法对软骨Integrinβ1、Col-Ⅱm RNA表达量有良性调节作用,这种作用可能与改善膝关节MCL的生物力学特性有关。Objective: To study the mechanism of treating knee osteoarthritis (KOA) with acupotomy therapy by observing the changes of biomechanical properties of medial collateral ligament and mRNA expression of Integrin β1 and Col-β mRNA of cartilage in rabbits with KOA. Methods: Forty New Zealand rabbits were randomly divided into normal control group, model group, acupotomy group and electro-acupuncture (EA) group. After modeling, the rabbits in the acupotomy group and EA group were treated for 3 weeks. The tension, creep and stress relaxation data of MCL were tested by Bose Electro Force 3300 protracted test machine, and Real-time PCR method was used to detect the expression level of Integfin β1 and Col-Ⅱ mRNA. Results: (1)The results of ligamentous mechanics showed that after modelling, every parameter of MCL was significantly reduced (P〈0.01, P〈0.05), after the treatment, those parameters all showed a tendency of rising, especially in the acupotomy group, the maximum load, maximum displacement and maximum strain all had clear differences compared with the model group (P〈0.05). Creep results showed that after modeling, there was a clear upward trend in the creep rate of the model group compared with the normal control (P〈0.01). After the intervention of the acupotomy, the creep rate was almost close to the level of normal group, while there was differences compared with the model group (P〈0.05). However, every biomechanics's parameter in EA group had no difference compared with the model group. (2)After modelling, Integfin β1 and Col-Ⅱ mRNA expression levels in model group were significantly decreased than thatin normal control group (P〈0.01). After acupotomy and EA treatment, Integrin β1 and Col-Ⅱ mRNA expression levels all showed a tendency of rising, with differences compared with model group (P〈0.01, P〈0.05). Conclusion: The method of treating bone by moduating sinews acupotomy has a good effect on expression of Intergrin β1, Col-Ⅱ mRNA
